MPS2-AS outperforms mpMRI in predicting GG upgrading in AS men

A multisite validation in 330 GG1 patients on AS found 9.4% upgraded to GG3+ and 37% to GG2+, based on systematic plus MRI-targeted biopsies.

The MPS2-AS test was superior to mpMRI in its ability to predict the biopsy determined grade group changes.

MPS2-AS is a urine-based test for detecting PCa upgrading in men with GG1 who are on active surveillance.

If these results are confirmed and the MPS2-AS test widely implemented, it could be a game changer for AS men, as it may reduce their need for repeat mpMRI's and biopsies.

This seems to be similar to the PSE test. I wonder if one is better than the other. They both seem to target the same thing.

This table seems to show the PSE test is more accurate.

If I had to go through the process of discovering cancer again, I would choose both the PSE test and the MPS 2 – AS.

After reading the Urology article it did make me wonder why it was a test for somebody that has already been graded vs the PSE test, which basically says you were likely or unlikely to have cancer.

I interpreted this study as one for those diagnosed with Gleason 3+3 (GG1), and are on AS, and wondering if a repeat biopsy is necessary to determine if progression to GG2 or GG3 has occurred.

For myself on AS with low volume GG2, I’m looking for something to indicate I’ve progressed to GG3, without having another biopsy.

If this test is better than a mpMRI and can indicate progression is unlikely…then there is no need for a repeat biopsy, at the that particular time.

Bottom Line: Those of us on AS need a biomarker test that replaces the invasive biopsy…this is a good step in that direction.
