metoprolol succinage and shortness of breatg in HCM

im a clinical pharmacist ( 45 years, board certified in geriatrics and also certified diabetes educator) and experienced increased shortness of breath with cardiac providers increasing metoprolol xl 25 mg to 2 tabs twice daily. instead of improving hcm complications this just made it worse, i could not hit 10 golf balls on the driving range without having to stop. I tapered dose down to 25 mg twice daily and all sob completely disappeared. if pulses are dipping into the 50s sitting and if diastolic is dipping into the 55-65 range and sob is problematic, lowere the dose.
