Labile Hypertension ~ How to improve it

Posted by jardinera25 @jardinera25, Oct 20, 2025

I have had labile hypertension since last December, which I believe was induced by my being put on two different Aromatase Inhibitors. I watched my BP go up and up, then it just went wacky and became spikes from 190/100 down to 100/60. I was switched to a different breast cancer drug. But now I'm left to deal with continuing blood pressure spikes, high mainly at night, a new problem. My meds have helped but now I can't take Nsaids, any corticosteroids since those will trigger high spikes. My BP meds have at times helped control it, but of late they put me in the low-lows so looks like I need a re-working of my med regimen. It's always changing! I'd love to hear any comments about others having this problem, and how you've dealt with it. TIA

Geez, @jardinera25 I sure messed up my comment above. I switched the tags around between you and @yellowdoggirl . Apologies!

Other than your blood pressure spikes I am curious how your labile hypertension is affecting you. You mention not being able to take NSAIDS or steroids. Are you needing them? Are you taking something different? What symptoms are you having?

Janell, the labile hypertension has affected my life more than even my breast cancer. A year ago when it started after several months on AI's I ended up in ER with spikes up to 190-220. They said heart looks fine, but gave me a prescription of Hydralazine for me to start to prevent the high spikes once I reach 170 at nihgt. I'm on 3 BP drugs, and the problem keeps evolving. My recent hip replacement surgery with its drugs changed how my meds work. In the morning I'm on the low side with dizziness, so had to drop some or it's feet up, on the couch, with too low BP. Lots more meds in the afternoon and strongest at night around dinner. Some nights I get lower spikes at midnight that go right back down, and others hit over 170 & I have to take the fast-acting Hydralazine. Am working with a Nephrologist to improve my nights.
Yes, I need NSAIDS & occasional cortisone shots for my Osteoarthritis (& surgery) but now they just drive up my BP into a spike, so can't use them! Even Nasacort is out. Tylenol is all I can take now for pain. Going to do a sleep apnea test in case that's part of the problem.

I ve had labile blood pressure for several years. Have been sent to ER for hypertensive crisis, but my heart checks out fine. I’m now seeing an endocrinologist and blood/urine tests indicate a possible phenochromocytoma. Small, usually benign tumor on the adrenal gland that can cause this. I’m having a CT scan next month. It’s worth a mention. Who knew why my BP went too high or too low. This might be the answer. It is rare.

I’ve had labile high blood pressure since 2018. My doc put me on a beta blocker which I still take. It still, sometimes, gets higher and lower, but usually my diastolic is way low (in 50s or even 40s, once 30s!) and my systolic is normal to mildly high (110-140). I learned that if there is a difference of more than 60 between top & bottom number it’s called a widened pulse pressure, which in my case turned out to be caused by leaky valves.
