Issues after Nissen fundoplication revision

Posted by refluxrefusenik @refluxrefusenik, May 23, 2023

Hello. I had a Nissen redo two years ago, and about a month ago I started to have some epigastric pain. It started with some epigastric pain sort of behind the base of my sternum, sort of like light stinging pain. I had some unusual throat tightness for a couple weeks followed by some unusually frequent burping. With both this redo and my previous Nissen, I could never really burp much, but that was always expected.
However, over the past month since the incident, I've been having mostly small burps (a few larger ones) close to a hundred times a day without exaggeration. Even if I drink water, I feel an awkward discomfort in my esophagus and have to burp, often multiple times. I don't feel like I'm having discernible reflux, but I do have a consistent discomfort in my esophagus. PPIs haven't seemed to help at all.

Has anyone else had any issues of temporary irritation where this has happened? I'm hoping it's a transient issue that could be linked to something else, but I'm not sure what else it could be. I'm in the process of moving and the company I worked at went under recently, so I'll be unable to see a doctor for at least a few months. In the meantime, I figured it would be worthwhile to see if anyone else who has had a wrap has faced an issue like this. I'd be especially curious to hear if anyone else had an issue like this and— if it went away after a time— what caused it.
The wrap was done by an experienced surgeon who said the durability should last for a long time, and I've been careful to not do anything that would cause too much abdominal pressure.

Anyone else have a similar issue?

@jlharsh I am scheduled for the scope in mid-April. I sometimes have quick sharp pain in areas of my stomach. I never had this pain until recently and although not bad, it is concerning.

I have not had fundoplication. I do have digestive issues and a scope was helpful to pin down stomach problems. I imagine April will not get here soon enough, but it does sound hopeful to provide some answers.

Please come back and let me know what you learn from your scope.

I am tagging @refluxrefusenik who I assume you are addressing in your comment. It is interesting that you have such similar symptoms. I agree, it does sound painful.

I had my Nissen many years ago back in 2007. It was amazing at first until I got the stomach bug. I literally vomited up my stomach.....
I herniated through my wrap and had to have it repaired. They decided to half wrap it and tack it to my diaphragm. That got me 6-8 months and it herniated again.... Reflux was terrible!!
I went back in and had is fully wrapped again. We'll guess what! It herniated again! I was doctoring elsewhere and decided to come to the Mayo. I was told to leave it until I have major issues or have a gastric bypass. That was in 2009/2010. Fast forward to today.
I ended up in alot of pain that I thought was gallbladder. I vomit every single day. Especially in the early morning. No I am not pregnant.
I am herniated more then I have ever been over 10 cm
I am scheduled to have the gastric bypass/reconstruction here in May.
I am scared but so hopeful this will finally help me. I am grasping this journey and am excited to do this and feel better.

Good luck with your upcoming surgery. I can’t think of a better place to have it than Mayo. I know if I ever have to have a redo, that’s where I’ll go. The surgeon’s experience with Nissen makes a big difference on the success of this particular surgery (from the research I’ve read). A former therapy patient of mine had a failed Nissen so I suggested he go to Mayo Jacksonville and he had a significantly better result.

Which Mayo Clinic will your surgery be done at? My surgeries were at Jacksonville.

Good thoughts and prayers as you go through your surgery and recovery. I don t know how I would have fared if I did not have friends and family having me in their thoughts and uplifting prayers in my behalf.

I’m new to this thread and have been reading everyone’s stories. I had my first lap nissen fundoplication done in February of 2015. I was 24 then. I had unbearable abdominal pain the first week after surgery. Horrible nausea that lasted maybe 6-8 weeks. I lost around 20 pounds in total. Finally after the first few months, I started feeling better. Before surgery, I had a hiatal hernia. For years I had issues with burping, reflux, regurgitation into my throat. I would swallow water and feel the need to burp and I’d burp it right up into my throat. Throughout the last 11 years, I’ve had a few scares that maybe my wrap had come undone. I would go through occasional spells of shortness of breath, inability to fully get out a burp. And for me, I was able to pretty much burp fine post surgery. And being able to fully burp was my sign that everything was working smoothly. When I couldn’t burp fully, I’d get this slight pressure build up around the lower sternum, and it would always kind of “correct itself” after a few days/weeks. The reason I’m posting is that 2 nights ago, I was eating dinner and tried to burp and I got a very sharp pain in my lower sternum. I didn’t think too much of it and went to bed fine. Yesterday I woke up feeling very short of breath..after I ate lunch, I had acid reflux symptoms. Burning in my lower sternum and throat. Which I haven’t had those symptoms since before my surgery. I didn’t fall asleep til 8 am this morning dealing with symptoms. So now I’m worried my wrap may have come undone. And the thought of doing this surgery again sounds like a nightmare. I’m planning on getting in touch with my surgeon to go over my symptoms and see about getting testing done to see if the wrap is still in tact. Just wanted to share!
