Is tymlos or evenity better for osteoporosis

I did not state that Evenity does not suppress osteoclast activity. I did state that it does not so to the extent of anti-resorptives. Big difference.

Hope others didn’t take it that way…I’m not a doctor and giving advice other than what I read…just relating my story and knowledge. I certainly am not infallible.

I’ll stay off the forum for now…unless something comes up. I don’t need the stress. It’s been a good place to get reassurance and info. I’m back to dealing with my husbands terminal illness.
Thx to everyone for the kind help you have been to me.

I am 73 and have been on Evenity for 4 months. Until 4 months had no problems then lower back muscle spasms set in! Would not recommend for 97 year old with prior breaks. They say it builds bones and the last month I can attest that my body is reacting (maybe building bones)!

Thank you for your advice. The fact is, Forteo is unavailable in Canada. I did like Evenity at the beginning but then read about the risk of strokes and heart problems. Now the only thing left other than the Prolia is the TYMLOS medication.

I’m very sorry about your husband. I went through that, too , few years ago. That is so hard.

Evenity (romosozumab) does transiently suppress osteoclast activity.
Here’s a more clear explanation:
Evenity is a monoclonal antibody that targets sclerostin, a protein that normally inhibits bone formation.
When sclerostin is blocked:
Osteoblast activity increases → bone is built faster.
Osteoclast activity decreases → bone breakdown slows down.
So Evenity has a dual effect:
Strong anabolic (bone-building) phase during the first few months.
Mild anti-resorptive (osteoclast-suppressing) effect throughout treatment.
How much does it suppress osteoclasts?
It does not suppress osteoclasts as strongly as:
Bisphosphonates (alendronate, risedronate)
Denosumab (Prolia)
Its effect is moderate, not complete, and is part of why bone density rises so quickly the first year.
Why this matters
The osteoblast surge builds bone.
The osteoclast suppression helps retain the new bone.
After finishing Evenity, most patients go onto an anti-resorptive (usually Prolia or a bisphosphonate) to preserve gains — because osteoclast activity rebounds once Evenity stops.

Is there a drug that once you stop you do not loose anything? Also, does Evenity make the quality of your bones better. An orthopedic surgeon told me these drugs fill the holes but make your bones more brittle in the long term.

Hi,

I was not aware that it would take 18 - 24 months before we see any improvement. What if she starts the Prolia and then goes through 12 months of misery with joint pain etc before we see any improvement? My Mother is extremely sensitive to pain so I have to consider the Prolia carefully. She had a fracture and is now in a wheelchair with physio 3x a week. The thing is, unless she can stand and walk again like before there is no point putting her on the Prolia or any of these osteoporosis medications. We live in Toronto, Ontario as well.

@norahguil I also had reaction to Prolia, but mine was dental. So I quit after the first shot. Having said that, given your mom's age, I think Prolia is an excellent choice. My sister has been on it for 10 years with excellent gains and no side effects. My brother also. Bonus of only being one injection every 6 months.
