@norahguil Why are you reluctant to use Prolia? At your mum's age, keeping her on Prolia for the rest of her life might help her avoid any more fractures. And it avoids daily injections which would be uncomfortable.
I also agree that Prolia should be considered an option. Being on it for the rest of her life eliminates the rebound problem when you stop Prolia. Do not think side effects are as likely widespread as other meds.
I’m not a doctor but I can see why at 97 they may not want to put her through daily injections that takes 18-24 months to see improvement.
What part of Canada? When I heard “Mum” I thought has to be Canada. My mother’s side is from the Ontario area.
They do things a little different there. The Evenity is suppose to build and prevent fractures within a year but not knowing all the particulars you check with the doctors…maybe ask for a second opinion if possible.
Thanks for the reply. We live in Toronto, Ontario. The Tymlos medication is not available here, whereas I know it is available in the USA and the EU. This leaves me with the Forteo or the Evenity medications. The Forteo Mum would have to start on prolia first before switching to Tymlos IF she cannot resists it.

Hi, well I have read a lot of reviews of people that are on Prolia and the reviews are more negative than positive. A lot of pain in the joints etc dentists are against the prolia too.

I was not aware that it would take 18 - 24 months before we see any improvement. What if she starts the Prolia and then goes through 12 months of misery with joint pain etc before we see any improvement? My Mother is extremely sensitive to pain so I have to consider the Prolia carefully. She had a fracture and is now in a wheelchair with physio 3x a week. The thing is, unless she can stand and walk again like before there is no point putting her on the Prolia or any of these osteoporosis medications. We live in Toronto, Ontario as well.

@norahguil
We’ll “see any improvement” is subjective…actually Tymlos and other anabolic start working or making structural improvements to bone in 1-3 months but to reach full efficacy is the 18-24 months …evenity is 12 months administered once per month. Thats very fast. Again the data, if you can believe it, suggests Anabolics first. Even my doctor tried to prescribe Prolia first. It is a slower acting drug twice a year not easy to get off of but has one of the largest usage data because so many women are on it. Again I don’t think there is any easy out here just I hope a little extra info from what I found out studying an awful lot of medical studies. I can see why Prolia might have been recommended but I’m only 65 and starting there just didn’t make sense.
Forteo has a long track record but why put her through that at this point. It’s a tough decision, I know you love your Mum, it must be very difficult…Hope you have some support. My husband and I are very isolated so all this health stuff can really get you down.
Ther3 are some very kind and helpful souls here. I imagine we all move on at some point in our treatment plans and maybe neve4 come back. I hope others in the same point in their lives find this archived info helpful.

Yes there is but over 26 million people have been or are on Prolia. Thats a lot! You Will obviously read bad reviews…everything has to be taken in to consideration and trying to use your best judgement is very difficult for sure.
I am trying to avoid Prolia as well but it’s because I’m too young…its a last resort.
@gently
I'm to start Tymlos early next year, so the news that it builds better bone than Evenity helps me feel better about both its more frequent injections (daily -- argh!) and its extra expense (Evenity is nearly free with Medicare Part B but Tymlos is ~$3K with Medicare Part D).
This is such potentially good news for me, I hardly dare to believe it, so I'd like to read up on the research. Can you provide a link or other reference to the source of your information?

This was my question for @gently (that is, what are your sources for Tymlos making better bone than Evenity).
@gently responded with a link to an article in nature.org entitled "Bone remodeling: an operational process ensuring survival and bone mechanical competence". You can google it
(I am apparently too new a member of this community to post the actual link).
I'm still working on understanding the article (it's very technical).
@gravity3
Generally forteo and Tymlos are anabolic…they work by stimulating the thyroid (parathyroid) that is partially responsible for telling the “managers” osteocytes to tell the osteoclasts to get to work building.
Some of these drugs like bisphosonates are just anti reabsorbitives that is they slow or stop the osteoclasts from cleaning old bone thus helping slow or reduce bone loss. Weight bearing excercise done with proper supervision has been shown to help stimulate bone growth without drugs. The issue can be is if your scores are really bad (and we don’t really have a readily available way to test bone strength thst all the doctors and research can agree on like REMS) then we just use DEXA scans. Not really the best but what is used.
So the reason you start with a bone builder is to build the structure of bone and keep the gains with a bisphosphonate …that’s what is called sequencing.
There are different approaches and success depending on you your body Doctor and your diet.
Excercise age…oh so many factors…it’s not as simple as just taking a supplement as some would have you believe…if you have really bad tscores you don’t have the time or risk that someone may have been able to successfully reduce their scores with…everyone’s body is different…
Sounds like you are asking all the right questions or as my professor used to say “there are no dumb questions”❤️
Evenity is newer and works entirely differently…it affects sclerostin and yet allows bone growth while building…that’s better in theory

Where did you find that Tymlos builds better bone than Evenity?
@norahguil
There is evidence now that an anabolic medicine is more effective when it is taken before a bisphosphonate.
