Full on menopause at 39. I began bhrt at 76. Estradiol and testosterone pellet, progesterone capsules and estradiol vaginal cream. Within hours I felt a strong surge of sexual sensation and voila....my libido was back. I attribute this primarily to the testosterone but certainly it also had to do with the estradiol. Best thing I have done for myself in decades. If only I had known and if my physician had not been in the thrall of the women's health initiative study. So many other benefits too.

A hysterectomy can cause less blood flow to these areas, which is necessary for libido and orgasm. For blood flow, I take a vasodilator, FDA-approved "Tadalafil" 2.5mg a day, or you can use a local vasodilator product like sildenafil cream (compounded for women) when needed. I also use an FDA estradiol 0.05 transdermal patch and local FDA vaginal estradiol, but these two routes of application did not affect my libido or orgasm. The majority of women need testosterone to restore their libido.
