How do I ensure my health history records are on file for first visit?

Mayo Clinic Rochester
Good morning. I'm wondering if I need to have all is my tests,x rays, ultrasound records... Sent out/with me when I arrive? I didn't think I had to because when I called the receptionist concerned that she can see all of my files on their computer system???

You mentioned you created list of previous medications, something similar for surgery may be helpful for quick reference.

The list could be quick reference and include:
date, type of surgery , reason, where done, surgeon

If your consultant needs more detail about specific surgery, they can dig deeper.

When I have something important happen, like ER visit or test, I send them via fax to my major Mayo Clinic doc. You can do this by calling their secretary and asking for their fax number. Also, when you check in for your appointment, they will ask you if you have any outside records with you. If you do they will scan them quickly and get them back to you.
