Has anyone had a Hysteroscopy biopsy vs a D&C for uterine biopsy?
Has anyone had a hysteroscopy done instead of a D&C for uterine biopsy after postmenopausal bleeding?
I am 74 and had two episodes of slight spotting. My gynecologist did an exam and I had a transvaginal ultrasound, both were normal so my Dr didn't think a biopsy to check for cancer was needed, but said she'd do a biopsy if I wanted one. I don't want the in-office biopsy because I've had that done before and it was very painful. She suggested the MyoSure procedure which is a hysteroscopic procedure done under sedation and is less invasive than a D&C. But, the MyoSure brochure says it mainly. used to remove fibroids and polyps. Also, some research I've done said "The use of hysteroscopy in endometrial cancer is not established as there is concern that cancer cells could be spread into the peritoneal cavity." Other studies say it's as safe as a D&C.
I want to ask for a biopsy for reassurance that the spotting is not due to cancer, but I'm wondering if a D&C would be the better procedure. I'm going to ask my Dr but wanted to ask if anyone else had the MyoSure procedure done after abnormal spotting?

Yes! Eighteen years ago I had two in-office D&Cs. They were incredibly painful even with Darvon and pain killers. I am not sure it was productive: I kept crawling up the exam table to get away from the doctor who was trying to obtain tissue samples. Fast forward to post menopausal bleeding last fall. The doc wanted to do an in-office D&C. I said “no-way” without knocking out me or the area. Dentists don’t drill on teeth without numbing the area!
So she did a hysteroscopy with a D&C in the hospital under sedation. Probably more expensive, but covered by insurance and very civilized - NO PAIN. Tissue samples showed Stage 1 endometrial cancer which was then removed with a robotic hysterectomy and determined to be Grade 3. Knock on wood - I had absolutely no problem with the surgery (walked 1/2 mile and attended a concert two days later) or the four VBT visits which followed. Go for the hysteroscopy with D&C over the D&C alone, especially if the D&C is to be done in the office setting.

3 Reactions@loislane1950 I can't understand how any doctor would perform an in-office D&C which means sedation is not possible. I had an in-office uterine biopsy which was very painful, so I can't imagine how painful a D&C have been. Sorry you had to go through that.

My only symptom is fluid on the endometrial lining and the thickening of the lining. I have no bleeding and no pain. I have a lot of bloating but that’s it. My GYN said it was ok to wait because I’m going out of the country. I hope leaving next week isn’t a mistake.
@reb2026 Thank you for coming back and letting me know about your plans. It's good that you discussed this with your gynecologist. What is the duration of your trip and when is your next appointment?
My DNC is scheduled for July 17. I’ll be back on June 29. I have several preop appointments before then. I leave next week. She warned me about the possible risks and that scared me to death so I said I wanted to wait until I get back and she said that was ok.
@reb2026 I've had two D&C's and both went just fine. I read it's considered a very safe procedure. Complications are rare; the overall mortality rate is about 0.6 per 100,000 procedures, making it safer than childbirth in some cases. Hope all goes well.

I try not to linger too much on those “possible risks” which to me sometimes seem more frightening than the actual procedure. But “they” have to inform us about all of these. Assuming you will be sedated during the D & C?
I had spotting issue, 2 weeks later ultrasound showed slight thickening, also had CT scan. I had D&C and then 2 weeks later hysterectomy, Good I got on top of it...Stage 1a Endometrial Cancer....might be slow growing, but I was glad I had done ASAP
