Do my blood tests results indicate MGUS or MM?
Light chains kappa 26.3 mg/l, lambda 11.0, ratio 2.39; gamma glob 3.7, M spike 3.5, glob total 5.7, mch 33.9, platelets 564, anion gap 9, protein total 9.6 g/dl, globulin total 5.6, and esp igm 4,655 mg/dl.

If only IgM is elevated, then Waldenstrom's Macroglobulinemia is possible. Rarer than MM, but much less aggressive. Many papers on it. Here's one on diagnosing it. Many hematologists may have never seen it. https://www.nature.com/articles/s41375-022-01762-3
@pmm At an initial workup by oncologist I had bone biopsy and body scan and many blood & urine tests. That was a benchmark exam. I will be seeing the same hematologist/oncologist in January. So we will see then.

1 Reaction@barbarian1 Bone marrow is an easy test these days. IT can be done with conscious sedation if you are concerned about pain etc. Surprisingly for me it was pretty easy and fast. Results clarify status . Sedation took care of worry and pain. Do not avoid test if your hem/onc recommends. IT is crucial to knowing where you are and then they have a good idea if treatment is needed or if you are a watch and wait. The results will not change for the better. They either stay the same or change not for the better. Do not delay. When and if treatment does become necessary an early catch and start are better than catching after progression to a critical stage.

Its not so much test but more so results and cancer treatment quality re potentially wrong diagnosis and subsequent treatment at my local hospital Im worried about. My medicare supplement ins plan requires in network oncologists and this hosp has only ave ratings for survivorship #s.
As I was filtering through the comments I saw that you were worried about insurance coverage. Don’t let that fear hold you back in finding answers. Just because the doctor is not at the Mayo clinic doesn’t mean they aren’t good. Just like God is Good so is knowledge. These forum are great for info and support, to bounce ideas off of others in the same boat. But it’s your paddle that will propel you. So keep reaching medical journals. I will say Your m-spike is over 3.0 which indicates that you have to take this seriously as well as your IGM. Did they do a 24-hr urine? What was your Beta-2? Did they do a skeletal survey? And I would do a BMB! the amount of info you get, mine was 11 pages long.
