Did BPH affect your orgasms, and did your procedure restore them?
BPH, Orgasm-Ejaculation, and Aquablation
Background: I’m a 66-year-old retired engineer, and my prostate was 45ml. Due to low blood pressure, I wasn’t a candidate for medication, so I opted for aquablation. I had the procedure on January 24, 2025, and as of today, February 11, I’m 18 days post-procedure.
Before the procedure, as my BPH progressed (weak stream, urgency to pee, and frequent nighttime trips to the bathroom), I also noticed that orgasms became more difficult to achieve. I estimate that I was only able to orgasm about one out of every four times, and when I did, it took significantly longer—often to the point of being uncomfortably long.
Since the procedure, my wife and I have had sex twice, and I’ve noticed that reaching orgasm has been much easier and faster. This has me wondering about others’ experiences, so I have two questions for the community:
1. Did BPH affect your ability to orgasm or the time it took to orgasm?
2. If you had a procedure like TURP, HoLEP, or Aquablation, did it restore your ability to orgasm?

I'm 61 and had HOLEP about 15 months ago. My BPH didn't affect my orgasms, but HOLEP has, in ways I am still not sure how to describe besides the 100% always retrograde ejaculation. Sometimes there's almost no difference from before, but sometimes right at the plateau before orgasm, that whole momentum just...disappears along with my erection. I can get it back a few minutes later, but it's frustrating. Other times, I lose my erection instantly after orgasm; pre-surgery it was usually a gradual detumescence over a few minutes.
The orgasms themselves can be very intense, but sometimes they're not at all. There is some feeling missing, and it isn't just about ejaculating. I think it's a kind of focus, but it kind of defies words. I needed the surgery, so this is the tradeoff. I wouldn't undo it, but it's weird.
Similar experience......at the point of orgasm, I have the "feeling", but no visible release of fluid....I miss that part. Orgasm now are good,, but dont seem to be near the intensity of previous, before HOLET. I had to have the procedure so I suppose it's a trade off.......
Early 50s with BPH since I was 37.
Everyone is a fingerprint. I was the complete opposite. I had retro once for a month or so from medication but no other issues. Since surgery , I say this humbly and grateful- everything is back to normal and better. It’s been 6 months now.
I have been hesitant to discuss this, but over the past couple of years my ejaculations have released less and less semen. Now(at age 77), I can barely get any kind of erection, let alone an orgasm. Even my libido has decreased significantly. Testosterone levels are normal. Up to about four months ago I could give my wife satisfaction manually. That was good for her and gave me some pleasure as well.
Now, I don't even think much about sex. It's not that I don't enjoy seeing sexy girls, but it doesn't really get me overly stimulated. It seems that my days are over per se. Any thoughts?
Feel same way surgery a year ago but even with Trimix and Bimix I can get an erection but fell nothing during sex not a hint of getting a dry orgasm , want an implant badly , no sex for a year is painful but if my nerves are shot implant wouldn’t be worth it and maybe my having sex with my beautiful wife are over as well
Try caressing, kissing, touching,etc. Slowly bring her to climax. Encourage her to dress mildly provocative. Tell her how beautiful and sexy you find her.
This may help you to feel better about yourself and also bring pleasure to her.
