@ginger38314 just curious if you met with radiation oncologists at OHSU and what that was like?

Had plenty of time to discuss my concerns and he was personable and
listened to me , one thing I forgot to mention if you will remove lymph
nodes during procedure.

@ginger38314 you mentioned that you had a low volume of cancer. Was it mostly 3+3 ?

Low volume meaning 2 cores 3+4 @ 10% component of 4 and .8cm lesion t1c.

@ginger38314 were you classified as "favorable intermediate 3+4" or unfavorable intermediate". I'm still confused by how somewhat favorable pathology can still lead to a high decipher score. But apparently, it can. Decipher in my case will be important as well in terms of treatment.

@ezupcic Pathology is looking at the cells in the tumor. Decipher is looking at the genes in the tumor. If they find BRCA1/2, PTEN, or other genes with high cancer risk that will result in a high Decipher score even if the current disease does not reflect advanced pathology.

@jim18

Decipher actually measures RNA expression of 22 specific genes. Those genes produce so called biomarkers that control cancer growth, signaling, metabolism of cells etc etc. The more mutations are present in THOSE 22 genes, the more is cancer aggressive. So it is different than any other test - somatic and germ-line look for BRCA etc. NOT Decipher.

@ezupcic

Pathology looks ate the shape, size and distribution of cells and Decipher measures genetic markers which are actually molecules (can not be seen under the microscope).

I was 3+4 favorable intermediate but high decipher which says I should get
treated in the near future and rules out AS even though I have only 2 cores
positive and a .8cm lesion and pet scan clear.

In some cases you can have 3+3 and a high decipher score also.
