Can you help me decide with neg ANA?
I am in my early 30s. I have been semi ill for half my life. Joint pain, chronic fatigue, hives etc. my PCP suspects Lupus so I was referred to rhumetology. I have a grade II diastolic dysfunction supposedly but cardiology wasn't worried about it before this. I have also been diagnosed with NAFLD.
Many labs were done. Most were unremarkable. Urine had a trace of blood this was negative for protein but a urine a month ago had a trace. Elevated liver enzymes
Ana negative
Sm Rnp 90
Anti SSDna 355
Anti Rna 22

Negative ANA doesn't eliminate Lupus, the other labs you listed are indicative as are the diastolic dysfunction, and liver disease. There is some exciting news on the horizon. https://health.ucdavis.edu/news/headlines/a-breakthrough-for-lupus-treatment-study-explores-car-t-cell-therapy-for-autoimmune-disease/2024/05

1 Reaction@ashrenee1111, I totally agree with @gently. Negative ANA doesn't rule out Lupus. Lupus is hard to diagnose.
Praying for you to get some answers. Blessings & Prayers....

2 ReactionsMine was negative and my rheumatologist diagnosed me with sjogren's disease
It’s not uncommon for the ANA to be negative and have direct antibody tests positive. Also, test sensitivities vary and antibodies are fickle! It’s why you need a rheumatologist to help interpret them. I had a negative ANA and a bunch of antibodies present by another method at the same lab. Follow up tests at another lab were all negative. Then a few months later the second lab got a barely positive ANA and direct antibody tests were negative! My symptoms were consistent with my first set of labs! It really makes you wonder if you have what they are saying…especially when it was unexpected! However when I think that another symptom shows up!
