Brinsupri and liver changes?

Yes. I posted recently about my liver enzymes being slightly elevated at my physical. Another person had the same thing. Read the prescribing information for Binsupri on the Insmed website about the side effect related to liver tests.
I am scheduled for some other tests to see if it's a problem.

Mine were slightly elevated (5-10% ALT/AST) but my lung doctor said still within OK range. I see my PCP in a few weeks and want to get his input on the trend.

hi iam looking for people who have been on Brinsupri for 6 months for there much appreciated feed back.Would like to know if it reduces muccus and coughing flareups.

@brody89 I have been on it since October 1, 2025. Mucus in lungs has been reduced dramatically. I only need to nebulize every 3-4 days & now only with sodium chloride & not albuterol. Mucus is thin & clear when I do nebulize & coughing is almost non existent. I have had no exacerbations since May, 2025, whereas the year prior I had 8 times when I tested positive for pseudomonas & was treated with Levaquin. I am hoping this progress continues.

That's encouraging that you've have such good results from 10/25! Have you had any side effects and with what results? Are you on 25 mg or 10.?Thank you.

@robot Thanks for your reply.Mucus reduction dramatically is what i was wanting to hear,which is fantastic.
My lungs airways were damaged from an inhalation injury and recently i was coughing up alot more mucus from the lungs.This drug Brinsipuri targets immune cells called Neutrophils.These cells release powerful enzymes that can damage airway walls.Brinsupri blocks and enzyme called DPPI which is needed to activate those damaging Neutrohil enzymes.
Iam located in Australia and cant wait for it to be approved here.

@brody89 best of luck to you - can you see if there is any research studies you can sign up for with Insmed (drug mfctr). i signed up for a research study here but was prescribed the drug before I started the study so I lucked out.

I think this drug is a game changer.The next stage is P63 lung basal stem cells which have shown great promise in lung air way regeneration.

@robot So encouraging that you have progressed to the point you describe. I would love the freedom to nebulize just once a day instead of twice. It's very tiring when I'm tired anyway. I would also like to know, as Ingrid1947 asked - are you on 10 or 25mg?

@ursala7 I am on 10 mg. just so you know I also have high blood pressure & take 3 blood pressure pills a day, stable stage III kidney disease, on synthroid for low thyroid, scoliosis which gives me considerable back pain, vocal cord dysfunction necessitating diaphragmatic breathing technique, and 77 years old. i have always been thin & eat well & exercise so everyone is different and genetics plays a big part in this as my grandfather & his 3 daughters all lived to be 102 years old. So I imagine not everyone will have the same results as me. I traveled to Denver to National Jewish Health last year for a one week medical review with Dr. Goldstein because of my constant lung exacerbations with pseudomonas. that trip was a life changer for me. I would hope everyone who reads this can get to a pulmonologist who is part of an approved bronchiectasis center, because that is so important.
