I take jardiance 10mg. Not a diabetic though. Was told it softens the heart muscles making blood flow easier. Here is some info from a quick dive into HCM and Jardiance.

Connection Between HCM and JardianceSGLT2 inhibitors like Jardiance show promise in HCM, particularly for non-obstructive cases or those with preserved ejection fraction and diastolic issues, even without diabetes. Key points:
Observational/Real-world data: A 2024 study (JACC: Advances) using electronic health records found that HCM patients on SGLT2 inhibitors (including empagliflozin) had:

Significantly lower all-cause mortality
Fewer hospitalizations
Reduced cardiovascular symptoms
No major increase in adverse events compared to non-users.

Smaller studies: In patients with non-obstructive HCM + diabetes, SGLT2 inhibitors improved diastolic function and lowered BNP levels (a marker of heart stress).

Preclinical evidence: Animal and cell models (e.g., iPSC-derived cardiomyocytes) suggest empagliflozin can reduce hypertrophy, improve relaxation, enhance metabolic efficiency, and potentially reduce arrhythmias in HCM-like conditions.

Clinical use: Some HCM specialists (e.g., at Mayo Clinic) prescribe Jardiance off-label for apical/non-obstructive HCM to lower filling pressures. Patient forums report individual use, often alongside other meds.

Jardiance is not yet FDA-approved specifically for HCM — its strong evidence base is in broader heart failure populations. HCM often overlaps with HFpEF-like physiology, which explains the interest. Important Considerations-Not for everyone — Benefits appear stronger in non-obstructive HCM with diastolic dysfunction. Obstructive cases may need different primary therapies.
Side effects: Increased urination, genital yeast infections, dehydration, low blood pressure, ketoacidosis risk (rare), etc.
Always consult a cardiologist (ideally an HCM specialist/center of excellence) before starting. It should be part of personalized care.

@mike223 ,
I have Apical Hcm and have been taking Jardiance for over a year now to lower filling pressures and help prevent afib breakthrough episodes. It has really helped. I also do not have diabetes. And yes, my Mayo cardiologist said it was off label. I am certainly glad I am taking it and pray it continues to help.

Jardiance made my EGFR decrease significantly and my Creatinine increase significantly so my doctor told me to stop taking it. Make sure you get your blood drawn to make sure the meds don’t have an adverse affect on other organs in your body. It causes me to be dehydrated.

My cardiologist put me on Jardiance almost 2 years ago it has been a miracle drug for me. I take 10 mg every day for my HOCM. My heart failure symptoms have disappeared since being on Jardiance. The best I have felt in years.
