alternative Evenity protocol

Has anyone used an alternative protocol to the typical 12 consecutive months of Evenity? One endocrinologist is suggesting using 6 month intervals with Reclast in between to maintain gains. He notes that during 6 months of treatment I would receive 70-80% of the gains possible and that doing 6 months at a later time would increase the possible gains for the 12 month total.

The idea of splitting 12mo Evenity into two separate 6mo courses to capitalize on its anabolic phase during the first 6mo is an interesting approach. The concern is that the bridging agent - Reclast - is a potent antiresorptive with prolonged skeletal retention, which could dampen the effectiveness of the second 6mo Evenity course. Has a shorter-acting bisphosphonate like risedronate, or even raloxifene, been considered as an alternative bridge?

Thank you for the thoughtful reply. I have the same concern and will speak with my doctor further. Thank you for the suggestions. Perhaps I should have shared additional information. I have taken one dose of Prolia, but am considering the course a new specialist doctor mentioned. He recently further explained that he would monitor bloodwork shortly after beginning Evenity and if there is a rebound effect after Prolia discontinuation, he would move to the Reclast. I am not clear about the odds of a rebound effect after one dose of Prolia. The Evenity would be timed for 6 months after the single Prolia injection.

@alwayslearning36, i see; that makes sense. Risk of rebound after one dose of Prolia is small but not zero. From what you've described, your doctor has a good plan for you, and you might have to use Reclast if bone turnover markers (CTX) rise significantly.

As i recall we have a member here who was in a situation similar to yours. She had a single dose of Prolia and then transitioned to Evenity while also starting HRT. She ended up with good bmd gains with that approach.

Do you know the endocrinologist that is suggesting this? I need to have my doctor take a look and I'm sure they will want to know where this is being done. Thank you!
