Muscle tears and PMR and/or pred?

Hi. I was diagnosed May and since that time I have tried to learn how to manage this bleedin' disease. My rheumy told to take 20 mg. for I think it was 3 or 4 weeks. Then things improved so she said I could go to b 15 mg for 2 weeks and then down to 10. Well things did not go well. She has been booked so I am doing my own thing. I did 20 again then 16 for weeks but symptoms would just hang on. Then I tried 17.25., finally 3 days ago I went to 20 again. It's helping some but I don't think enough. Today ended up being a very painful day. I might go up a few more mgs.
So this is my question...last week I was doing something minor And moved my arm and felt seering pain in my bicep. I could hear and feel something rip. I saw stars. Then two days ago I leaned down to pick something off the floor and same thing happened in the back of my left thigh talk about painful!!..today I went out on the porch with dog and.damn I took a step and once again felt that same thigh muscle kind of rip again. I was so mad. Ever since I was sent on a fast.pred reduction I have been in so much pain and so exhausted that I am not doing much. So have any of you experienced this? I know pred is hard on your body...and so is PMR... Duh! But .... I take.magnesium everyday and have do so for.more than a year. I take a multi vitamin as well. So...any experience, ideas, treatments...pls let me know. Also anyone heard of MSM? And last...I am thinking of increasing fr 20 for a bit to see if I can get more than two decent hours in a day....not including my muscle rip days. Thanks everyone.

I started 20 Monday. So we will see. I just realized that when this all started in May and she put me on 20 I was already on 3 --
had been for years for sjogrens RA. So maybe I really need to go to 23. Anyway...the post was really more about the muscle tears. I gave the background to help with answers. Anyway just curious it is not fun
..my leg is not happy this am.

Based on your description & my fun world of so called "muscle tears," I'd guess because of proximity, it's PMR pain. I'm amazed at what I can do to increase the pain. Hang in.

Mine started with a hamstring tear, had had some shoulder pain for a year, osteo arthritis that became inflammatory. After the hamstring got better and I became active again, full scale PMR hit both hips in the groin, a lateral; bursa where the hamstring was and both shoulders. This was in late December, by Feb 10th the hips went bad. I thought I needed 2 hip replacements!

Hi there, the same thing happened to me.I pump fisted after a playoff hockey goal,instantly felt pain in my shoulder,and went to Dr.
It was a muscle tear.Clinic gave me two shots of Cortisone, better in a week.
I have had Polymyalgia-rheumatica for 2 years, am on 5mgs.
The answer is enough Prednisone to reduce pain and taper slowly,good luck.

I have not had a feeling of muscles tearing but I have had sudden stabbing pains in my biceps and thigh. This leads me to a question which maybe I should have started another thread on. Someone, I think here and perhaps one of the moderators, posted a link to some new research on the cause of PMR. The research showed that it may be inflammation of the sheaths around the muscles rather than in tendons and bursae that leads to PMR. I should have saved the link but did not. If that's the case it could explain a tearing sensation and also many of the symptoms I have had that seem muscular, not in the joints.

@linda7 "UP to date" is a site that a colleague's father , who is VA doctor, shared with me . This section mentions hamstrings twice :: i wonder how invariable is the bilateral nature of all of these findings.
Clinical manifestations and diagnosis of polymyalgia rheumatica::
Imaging findings at the shoulders include subdeltoid/subacromial bursitis and biceps tenosynovitis. Bilateral subdeltoid/subacromial bursitis is an imaging hallmark of PMR; a meta-analysis of four ultrasound (US) studies found this abnormality in 66 percent (95% CI 36-87 percent) of patients [23]. At the hips, PET studies have demonstrated increased uptake of 18F FDG at the greater trochanters and ischial processes, attributed to bursitis, iliopectineal and iliopsoas bursitis, and hamstring tendinitis. MRI and PET CT have shown evidence for interspinous bursitis in the cervical and lumbar spine (image 1). Glenohumeral and hip joint effusions are less frequently observed.

Upper-extremity symptoms in PMR thus result from biceps tenosynovitis, subdeltoid/subacromial bursitis, cervical interspinous bursitis, and, to a lesser extent, glenohumeral synovitis, while pelvic girdle symptoms arise from involvement of various bursae about the hip, hamstring tendinitis, lumbar interspinous bursitis, and hip synovitis.

My hamstrings are on fire. I assumed it was the little exercise I’m trying to do. I. Can no longer take my walks or play golf. PMR is taking over my life.I tore a muscle in my calf just from walking, so this has to be from PMR. I’m in good shape for 83, but now I feel every bit as old! I’m not giving up though, I hope the medication helps.

I ruptured my bicep tendon about 5 months after my PMR diagnosis while playing golf. It was tore at the top of the tendon so I just have to live with the bicep deformity for the rest of my life. This has been extremely frustrating as I am in very good shape and participate in vigorous exercise. I’m sure this is a result of taking prednisone. I hope you only have a tear and not a ruptured bicep!

@sandmason, If you only have 5 mg prednisone tablets, you might want to see if you can get scripts for 2.5 and 1 mg tablets. The smaller dose tablets really helped with my taper.

Thanks, I got 1mg to taper to 15, where I am now, will need many more as we go. Like why no refills? Run out of ones, and as I cut corners off pills I mix the ones and 5s I cut and now dont know how many mg I have left over. Grrr.

Your doctor or rheumatologist should refill your prescriptions if you ask. Mine gave me a 3 month supply of each 2.5 and 1 mg tablets along with the 5 mg tablets. I would ask them and let them know it's so you can taper at a lower dosage.

Watch your diet and research the foods to avoid and those that help PMR. I found I started to recover after I did this. Don't push yourself. Take time to rest during the day. It's a long term illness but it probably will come to an end. (I say probably as I am not sure about your situation, pretty much everyone get's over it eventually).
I too am glad I was in good shape for 74, four years ago, and that being stuck at home during Covid was no problem. We have come this far and achieved so much that we must be strong even if there are days you don't feel that way. Don't give up hope, pamper yourself you have earned it. Good luck.
