Thrush after anti-rejection treatments: What helps?

Posted by jbaggi @jbaggi, Nov 7, 2022

I had to go through two rounds of anti-rejection treatments this summer for mild rejection. I finished the last treatment 4 weeks ago. Now I have a case of thrush . This has lasted for 3 weeks.
I was taking nystatin during the treatments. Now I’m on 100 mg of fluconazole every day for 10 days. It seems to be getting worse.
I do have lupus, and have had it for 42 years.

I am very hopeful that this biopsy will finally show that the mild rejection is under control now! Please post after your biopsy. I am looking forward to my BK virus resolving pretty soon too. Getting the balance between rejection and infection is an ongoing process. But definitely worth it! Talk to you soon 😊

I had Thrush once, and when I swallowed, it felt as if I was swallowing broken glass. Awful, and brings tears to my eyes just thinking about it!
As a transplant recipient, I am taking immunosuppressant meds. I also have an albuterol inhaler to control diagnosed Exercise-induced asthma.
My throat was sore after a long flight home, after a trip to high elevations in one of the National Parks where my husband and I spent days on the trail. I had incorrectly decided that my sore throat would go away after a few days of rest and hydration. MY 'self diagnosis' was wrong and as always happens, it was worse at night . So I went to the ER, where the nurse said that the cool dry air, the altitude, and the additional need for the inhaler while hiking had created a 'Perfect Storm' scenario for the thrush to develop. I was prescribed the Nystatin in the oral swish-in-mouth for 5 days. After 5 days I went to my PCP to get it checked, and he prescribed another 10 days. It went away, and have not had another incidence of it since.
I notified my transplant dept of the medication as soon as it was prescribed. I don't recall the incident having any effect on my labs.

I just had a liver transplant and having a hard time taking the liquid thrush medicine… any points on how to make getting it down easier? Did you take it right after transplant?

@ecinet777 - welcome to Connect. congratulations on your recent liver transplant! When did you receive your transplant? Mine was in 2009 and I honestly don’t remember having difficulty with the medicine that I was prescribed for Thrush that was around 10 years after my transplant. I also don’t remember what I took after my surgery. I think there was an oral swish-in-mouth med. and I don’t remember that either.
I am sorry that I don’t have any experience that might help you. However, I do want to encourage you to contact the doctor (via your transplant nurse coordinator) who prescribed the medicine. It is important for you to take all of your meds as prescribed.
How many more days of the med will you need to take?

Hi @ecinet777 😊
It's great to meet you! Congrats on your brand new liver! Are you still in the hospital or back home now? I am a two year post kidney transplant patient. I remember having a problem with the Nystatin "swish and swirl" immediately after the transplant. At the hospital, I would focus on swallowing all the immune suppression meds, the antibiotic, the anti-viral, and by the time I got to that terrible tasting "swish and swallow" anti-fungal, I was getting nauseous. The first two or three days, I told my hospital nurse that I could swish, but not swallow without gagging. After I left the hospital, it started to gradually become easier. Maybe speak with your nurse to see if you can use a little less for the first couple of days, or some other revision. You definitely want to protect yourself against thrush in your mouth, but I understand what you are going through immediately after surgery. It's tough to "swish and swallow" that taste after taking all those new pills. But it definitely gets easier as the days pass, so hang in! It was a prescription for only 30 days. How long do you have to take it? Again Congratulations on your transplant and post how you are doing! 👍

I had a culture which came back negative for yeast. I called my ENT doc and sent him some pictures. He diagnosed me with white tongue.
No real treatments other than sipping water, good oral hygiene etc. Doc said it can come and go.
Probably caused by my lupus and all the drugs I’ve been taking for my rejection episode. In addition I got Covid for the first time. Sure that didn’t help.
I have my 3rd kidney biopsy since June on the 30th of this month to see if the treatments worked.
Thanks to all of you for your helpful comments.

I was diagnosed with thrush after my transplant at Mayo Rochester. I was given large round, thin, flat tablets to suck on or chew slowly. I don’t remember the name, but I took them for several days. They worked.

@ecinet777, Are you still taking the thrush medicine? Is it getting easier for you?
I hope that your recovery is going well. Are you at home?
