Pain in the butt - Can't sit down
I have had very intense pain at my sitz bones for over 7 months and am unable to sit down. when I sit, it feels like I am sitting on two hot pokers or rocks and it is really unbearable. So, I have been standing for seven months. I've been to a couple of spine and pain management doctors, an orthopedist, a chiropractor, a neurologist, an acupuncturist, and a massage therapist. The original diagnosis was ischial bursitis but that ha not appeared on any images. I don't have any issues with any of my lumbar disks. I'm in constant pain even when standing or lying down though those activities do not hurt as much as sitting. Has anyone heard of anything like this before?

Do you think you have deep gluteal syndrome with extra-spinal entrapment of the sciatic nerve? That can cause pain upon sitting. The most common form of entrapment is piriformis syndrome. Often, piriformis syndrome can be detected on a pelvic 3T MRI scan. It's challenging to find physicians outside of academic medical centers who have expertise with piriformis syndrome.
@jim19608 - You mentioned your pain is from the ischias bone- not the coccyx- tailbone? Did you have X-rays or CT/MRI before the physical therapy?
Yes - same thing but maybe not as intense. Had for years.
Tried shockwave, various injections, physical therapy , acupuncture, massage etc and nothing works long term.
Oh! Ouch! Is sitting easier without a tailbone? Have tail bone pain but no diagnosis of anything especially when I mention that both hips have had total replacements and revisions due to Ehleurs Danlos not permitting calcium into bones to cement implants. Sometime it hurts when lying down, no pressure. Good luck! Maybe there needs to be better insight into the control of inflammation
At the risk of you thinking I’m nuts I’m going to recommend something to you because it sounds so simple. If it doesn’t work you have nothing to lose. I buy these flannel wrapped ice packs from Amazon…they do not drip. My recommendation is to ice often the area. I have suffered with pain for years and this has helped me very much. The name is Core Products Dual CorPaks hot and cold packs. No matter what God bless and good luck.
I have had a similar problem since March 2021. Trying to fix another problem, an orthopedic surgeon recommended specific hamstring stretches – told me they would be hard and likely hurt. Unfortunately, this created bilateral tendonitis at the insertion of the proximal hamstrings into the sitz bones. I have had ultrasound and MRI imaging to confirm this. I have been through extended physical therapy, ultrasound treatments, platelet rich plasma injections; and still the pain persisted.
My best relief is a wedge pillow that I take and use everywhere, plus at least a soft bed pillow on assorted comfortable chairs around the house. I continue a rigorous home exercise program modeled on my physical therapy and am now at around a 50% pain reduction after 18 months. I use TENS when the pain gets bad, daily Tylenol, and Nabumetone (as needed).
I am hoping that my (slow) progress continues, but this has been a challenge. I am a 78-year-old, Stage IV Lymphoma survivor and I suspect the side and late effects I have from this and the treatments I needed (spine tumor surgery, inpatient and outpatient chemotherapy, and radiation) have made it harder to heal from this, and other, types of injuries. And I do take Lyrica for the neuropathy the RCHOP caused.

Have you tried setting food allergy testing - specifically testing for inflammation markers? I found that while I'm not allergic, eggs cause this same pain for me a day or so after eating them.
