Protect Your Vaccine Record Card

I was just looking at my half-finished vaccination card. Next Sunday is my second vaccination. I can't wait. It's been well over a year that I've been out and about as most of us.

But hold on, what do I do with my card once I'm vaccinated? CNN has some wonderful tips.

First off make sure that all the information on the card is 100% accurate BEFORE you leave where you are getting vaccinated. (name, date of birth, and the date and location of the vaccine -- for accuracy. )

Secondly, take a photo of both sides on your phone, for the first vaccination (if possible) and then the second. Keep a copy on your phone and email a copy to yourself. Then photocopy it and keep at least one copy with your important documents.

Lost cards or very damaged ones need to be replaced. Contact the provider of your vaccinations if you need to replace yours. You might want to jot down the time and place of your vaccination in case you lose your card and need to replace it. Visit your health department immunization information system (IIS). My state has a COVID-19 Vaccine Portal through the State Department of Health.

After receiving both doses of the Moderna vaccine does Mayo issue CDC ID cards verifying patient vaccination status? If not, how can you get this document?

Hi Mayo Connect- with the increase for vaccination confirmation for things like travel and concerts, etc. Is there any reconsideration of issuing the CDC vaccination card, and if so would you retroactively issue for previous vaccinations? This is becoming the gold standard....

@woodwick29- Good morning and welcome to Mayo Clinic Connect. The card that you were issued is a CDC vaccination card.

I believe that your pharmacist or PCP should have information about your other vaccines. This is a great question. I'd also like a record of all of my vaccinations!

I did not receive a card when I was vaccinated at Mayo. Instead, they printed my entire vaccination records. I would expect that locations requiring proof of vaccination would expect something closer to the format of a CDC card versus my vaccination records. I'm concerned that showing my COVID vaccination record from the Mayo app will not be acceptable for these events.

I agree. Go to VAMS and you can get your card. Will you let me know if it was easy or complicated?

Correct - I was not provided a card but a print out of my vaccination. It is not in the same format and I am just concerned it won't be accepted or it will be a headache and a half getting it accepted. I was vaccinated at Mayo Clinic in Red Wing MN. Is it possible to get a 'replacement' card?

I was able to create an account on VAMS, but it will really only let me schedule an appointment - doesn't seem to have any ability to look up my vaccination. Am I missing somethign?
