Protect Your Vaccine Record Card
I was just looking at my half-finished vaccination card. Next Sunday is my second vaccination. I can't wait. It's been well over a year that I've been out and about as most of us.
But hold on, what do I do with my card once I'm vaccinated? CNN has some wonderful tips.
First off make sure that all the information on the card is 100% accurate BEFORE you leave where you are getting vaccinated. (name, date of birth, and the date and location of the vaccine -- for accuracy. )
Secondly, take a photo of both sides on your phone, for the first vaccination (if possible) and then the second. Keep a copy on your phone and email a copy to yourself. Then photocopy it and keep at least one copy with your important documents.
Lost cards or very damaged ones need to be replaced. Contact the provider of your vaccinations if you need to replace yours. You might want to jot down the time and place of your vaccination in case you lose your card and need to replace it. Visit your health department immunization information system (IIS). My state has a COVID-19 Vaccine Portal through the State Department of Health.

I had shared this earlier, however, in the event this is helpful, the card was created by the CDC to be a helpful reminder and a way to confirm that the correct second dose is provided to you. At Mayo Clinic, however, the card is not necessary because Mayo is able to track when you should return and which vaccine you will need for your second dose. Should you have a need for the information on the card, it will be made available in your Patient Online Services account under immunizations.
Also, I have been told that you can request a physical vaccine card when you go into Mayo to get your shot. They aren't handing them out unless requested in order to create efficiencies due to the information being available in your Patient Portal.

I have read that you shouldn't laminate because there may be a need to write in boosters in the future. We made photo copies of our card and put them in our medical file. We made sure our primary doctor's office added out Covid vaccines into our immunization record. The original card we file with our passports.

I was at Mayo this week and they already had my COVID immunization information from Walgreens. When I checked my immunization under Medical Record on portal, it was there.

@roch - I texted my doctor through my patient concerning notification of my vaccinations. They also got a notification that I was fully vaccinated. I bet my Walgreens has it too.
I received my COVID vaccination in January. EU countries are requiring a CDC vaccination card for proof. Will Mayo Clinic be providing cards for vaccinated patients?

Clinics which administer the vaccinations issue CDC cards at the time of the injections. If you didn’t receive a card, contact your medical provider where you received the vaccine.. They will have your immunization records on file and should be able replace your card.
Did you receive your vaccinations at a Mayo Clinic campus? If so, you should be able to access your records in your Mayo Patient portal and make a copy of the information. Contact the Mayo Clinic where you received the vaccine to get a sanctioned CDC.
