Gout meds to lower uric acid and chronic kidney disease

I am Stage 4 CKD and have had a few episodes of gout. Is anyone on this forum who is in late stage taking or has taken meds to help with lowering uric acid? Thank you for any responses, positive and negative.

Hello @kitmus. I have CKD stage 3B and gout. Through Mayo telemedicine I was given allopurinol and colchicine. It appears that these are the standard prescriptions. I was also given prednisone for acute attacks. Due to other medical conditions, I had issues with the allopurinol and colchicine. Then I was prescribed Uloric, but still had a reaction. I finally got my Covid vaccinations, so I am returning to Rochester in May for testing, examination, and treatment. I was told that the plan is to reduce uric acid below 6, because that is the level at which the crystals that create the gout are formed. I hope you find this helpful.

Thank you. I am now on allopurinol daily. Am also on colchicine low dose for thirty days as “cover”. My feet are better.

@kitmus Drinking enough fluids for you. Some drs now say we should aim for 100 oz of fluids per day. Drink before you feel thirsty. I am known to always be drinking water, even keeping a bottle next to the bed "just in case". Hydration levels are important!

I am told to drink 3 liters a day. Drinking enough is not an issue for me. I love water, btw.

Sorry for my delay in responding just haven't been keeping up. As far as how I came up with the protocol. I have a Master of Nutrition degree and I spent a lot of time looking at the research. Your taking the Knudson's is fine but I have read studies that the Montmorency type of cherry from Michigan is the most beneficial based on nutrient content. I use Obstbaum Orchards concentrate which is much cheaper and can be bought online. It also has a longer shelf life and I just use it full strength in smoothies or it can be reconstituted back into juice by adding water. I know from my experience when my uric acid levels were 14-16 I had many more gout episodes. Touch wood I am now 6-7 and can't remember my last episode. So I do see a correlation with me as far as uric acid levels. All in all the protocol helped to start but when I found out the CKD and started to limit my proteins, especially from animal sources which I still eat 3-4 times a week, and reduced my sodium things really improved. Also my eGFR and creatinine levels have improved as well.

I have begun looking online for the the ingredients in your protocol. You have obviously put a lot of work into it. It’s impressive. I plan to get the Michigan cherry 🍒 extract right away. I think you’re right about why that’s a good approach for cherries as opposed to the one I’m taking. I really like your other ingredients too but plan to hold off until I have more labs, testing, and exams in May.

It’s gratifying to know that you’ve had such great results as your own best advocate and that your doctors support you.

Better feet is good news. Hopefully those meds will do the trick for you. Stay on top of your blood results and keep in touch with the doctors, so they can monitor your progress.

Thank you, @gladiator. I would have thanked you sooner, but I’m still coming up for air after a couple of 25 hour days. And here I thought I was retired.

I will definitely look into your recommended online sites as well as the celery seed. I really appreciate those tips. I’ll be at Mayo Rochester in a couple weeks and hope to get some more answers. It appears that I may be blessed with more issues than CKD and gout.

Here’s hoping for your continued successes on your positive trajectory.

I wish you the best at Mayo. I don't make the trip back until the fall. I also have a multitude of issues that have arisen in the last few years. I have OBD. This is what a good friend of mine calls it. Old B**tards Disease. I can't find anything in the literature but I guess it fits some of us old guys. I have been fortunate that when I started to restrict my protein intake my Uric Acid levels fell and my gout issues declined as well. But I still do my protocol just in case. I wish you answers to your issues.

I have CKD and I’m near Stage 4 with eGFR of 34. I developed Gout in September of 2019. My PCP gave me Prednisone for it (20mg for 3 days, 10mg for 3 days, then 5mg for 3 days). He wouldn’t prescribe the other Gout Rxs because my Creatinine Clearance was 51. After the second attack in December 2019, he started me on Allopurinol 100mg daily. After I had a total of 5 attacks within 10 months, he upped my dose of Allopurinol to 200mg. The increased dosage caused my legs and feet to go numb 2 hrs after I took it. My Nephrologist said that Allopurinol was causing Peripheral Neuropathy and that it was not good so he switched me to Uloric (Febuxostat) 40mg a day. That stopped the Gout attacks. It is expensive and BCBS wouldn’t approve a Prior Authorization for it, so here’s what my Nephrologist did: He wrote the Rx for 80mg (which I cut in half) for #90 and gave me a sheet for Costco Pharmacy which said it would cost around $165.00
which would last for 6 months (so it equals less than $30 per month). My local pharmacy agreed to give me the same price as Costco. I haven’t had any side effects from it, and my Uric Acid went from 5.6 (which still caused Gout) down to 2.4.

Thank you so much for your feedback! I am now On Allopurinol, 150 mg. Have not had any gout, but continue to have painful feet. I will check with my neph on this other drug. Interesting and informative and creative prescribing!
