LVOT Bigeminy with NSVT/PSVT

I have had Bigeminy in the LVOT that is now coupled with NSVT and PSVT going on two years. I had an locally unsuccessful ablation in 2020. I am getting ready to come to Mayo Clinic for a new ablation procedure in April 2021. My question is are there others that have arrhythmia like this without known heart disease or blood pressure issues and what was their experience? I am a bit nervous I have been reading about the risk of left sided heart ablation and I am a bit scared.

I assumed not too many have what I have the Bigeminy is similar to a sustained Tachy as it can cause the heart to stop. That is my understanding. An episode can last as long as 3 hours. My tachy is very short but new.

@paula3166 Interesting, i assumed when you said arrhythmia it was a fast one with me it would go up to near 200 and effectively stop moving blood.
With yours it is slow so your stopping is more because of a low heart rate im guessing? I just never heard it called an arrhythmia before. So im not sure how much I can help further except I have had both internal and external ablations and can maybe help with the process of an ablation. When ever I had an episode tho it was more urgent. Like it need to be solved in under a minute or so which is why I had a difbulator also.
Well I'll be available for questions and look forward to hearing more once you find out. The Heart is an amazing organ but also Mayo is on the cutting edge of research in many areas. I'm pretty confident they will figure this out.

Just as an example unrelated to the Heart, I developed a fever mid 2019 and was hospitalized due to low blood counts. I realized first hand how Mayo Doctors don't give up. They literally tested every part of my body to find the source of this fever, normal blood test could not find it and I was simultaneously dealing with some other viruses, so I was there 5 weeks and finally they found a virus in my lungs that doesn't get into the bloodstream. The only way they found it was to put a scope into my lungs with a rinse then removed the solution and that's when they found it. A simple antibiotic took care of it. But my point is they are determined to help and won't stop until they do what they can. It helps a lot that they have staff doctors of almost every specialty so it makes diagnoses much quicker than other hospitals I've been too.

What an amazing story of Mayo's diligence in finding an answer to a hard-to-solve problem. I hope this will be an encouragement to @paula3166 and to others going to Mayo looking for answers to their health problems.

Thank you for your reply most times they call these ectopic beats until they start a Bigeminy or trigeminy pattern the. They reclass them to a tachy although it is slow if you have more than 3 in a row. I have episodes that every beat is followed by a misfire that can last 3-4 hours. My mother had AFIb at the end with a pacemaker so I understood lots of the heart at the point I started having problems. There are drugs to slow the heart down and most arrhythmia meds target slowing it down. They don’t work on someone dropping to the lows I see it makes it worse. I had to do a lot of research and understand what was the issue. The multi focal part means several signal origins which means the left ventricle may be unstable snow I say may because not all the signals might originate in the left ventricle. I have seen tachy though which when it showed up was two years into the Bigeminy and the thought from local cardiologist not Mayo is that the Bigeminy triggers it. I am no heart expert but I have learned enough over the last few years. I know dehydration, certain meds like Valium class drugs, tired, exercise, not eating enough, weather change, and many other things bring on an episode. I also have a standard low pulse in the 50’s for unknown reasons.

Thank you for telling me you have had several ablations and did just fine I know you didn’t state if they were left ventricle or not I know the first one I have in right ventricle sent me into Bigeminy for two straight days I was miserable.

Yes, you are right. Your concern/worry is very normal. Most of us would feel the same way if we were in your shoes. An MRI of the heart is considered a great way to look at the heart. Here is a very short video from Mayo that might help you see what information a cardiac MRI provides to you and your doctor.

I had to go look up my Mayo doctor his name is complicated so I had to look at registry. Dr Samuel
Asirvatham He is the manager and there is an associate as well do not know that name. Dr A as I call him has gone on a long journey with me we have had to reschedule a couple times because of extenuating health issues that required delay. I have had a bout or two with blood peripheral eosinophilia. They have stayed on it and refused to let me back out which I admit at times I have wanted to. I have gotten used to doctors saying I don’t know what is wrong that I just accept that and deal with it.

That story makes me feel good I have had so many odd things that I just deal with because my doctors locally can’t figure it out. I am hopeful that once I am there they will look at me as more than this one issue. I have had to delay because of eosinophilia issues and they have kept on me to come. After 4 doctors locally saying Mayo is next I am finally going. Your story gave me hope

I've had if memory serves about 7 or 8 ablations I've lost track 6 for sure with 2 of them external to the heart. And based on using both sides of my groin area during one 5-hour ablation I had, im figuring both sides were ablated. That one was my most successful 5 hours on the table and 5 separate catheters to access different areas. But I had a pretty sick heart from a bad flu bug I got one year. If you haven't figured yet I ended up with a transplant. Doing great now but transplants come with different set of issues. Nothing drastic just different.

It’s six hours away I will have to stay at a hotel most of the week. Except the procedure day and they will keep me overnight if I am stable back to hotel for another two days

That sounds scary a year into this heart issue my liver went into full hepatitis without known virus so I am assuming at some point either heart or liver may put me in a similar situation.

@paula3166 Please realize tho I had a great life during about 8 of the 9.5 years of Heart issues and now 3 years since transplant my life is pretty awesome
I mention that so for me there were options and im sure for you also. During that time I worked full time and did pretty much anything I had desire to do. Hiking, camping and a physical job I really liked so ablations along with meds and a pacemaker delayed an major issues for almost 10 years and like i said my heart was at 5% EF. Plus I did not have Mayo until the transplant so your ahead of the game already.
I'll be thinking of your procedure and add my thoughts during my prayers.
