What is this in my urine?

So for 4 years I have seen 4 different urologists and suffered with an unceasing feeling coming from my urethra that I have to urinate. I now have pelvic floor dysfunction. All my urine tests have came back negative and I have probably had around 20. With this said I have a bad smell smell to my urine and particles/bubbles(these don't go away) present that were not there prior to my health condition. I attached a picture. Does anyone have any idea what this is?

I am a 31 year old male. I have had 2 bladder scopes that revealed no abnormalities. The pelvic floor dysfunction is a total mystery and slowly developed from the start of the chronic urination until it became severe. The only abnormalities on my pelvic scans were seminal vesicle cysts and red bone marrow reconversion. No doctors gave this any serious thought and dismissed both of them as red herrings. So basically I have significant side effects with zero clue of why.
