Decided on nerve sparing surgery

Posted by ginger38314 @ginger38314, Sep 30 8:23am

I decided to go the surgery route, if it wasn't for the high decipher score I would have chose sbrt, so they will call me within a week to sceduled, thanks everyone for you comments.

@surftohealth88 can the decipher score change the grade group and staging classification determined by a biopsy?

@ezupcic
No - Decipher only gives information about aggressiveness of the cancer - how "eager" is the cancer to escape gland and produce mets. and how much did cancer cells change genetically in comparison to normal cells. It looks at RNA biomarker changes.

@ginger38314 AI says I have a 75-80% chance of a low decipher score based on my pathology. Probably overly optimistic.

@ezupcic
If you are talking about AI -it is not. AI works with what you "put in" as an information. It can not extrapolate.

If you are talking about yourself - that is fantastic !!! Being an optimist by nature will help you immensely with enduring this saga and all of the treatments 👍😎 and keeping your sanity intact.

Mine was about the same I think it was a 18% chance of a high score and
ended being high but luckily it's contained. Originally was going to do
sbrt but because of score decided on surgery have a tentative date in mid
February and I'm also on cancelation list. Best of luck.

@surftohealth88 AI response was based on my entire profile and pathology. I’m not as optimistic but I’m trying to maintain some hope.

@heavyphil if a 3+4 with a low % of 4 metastisized wouldn't it have other features that would make it more aggressive? I agree that the favorable/unfavorable distinction is confusing. At the end of the day it seems like all signs point to either radiation or surgery.

@ezupcic Yes, it definitely would. If you go to the Veracyte website, the use of Decipher is explained.
Go to the videos to learn more and click on ‘use in intermediate cancer’. The oncologist talks about exactly what we’ve been discussing here: ‘favorable’ isn’t always what it seems and can metastasize, while ‘unfavorable’ may be indolent and remain in situ…
As for Dr Scholz’s input on this, please remember that Decipher is basically HIM in digital form; you really don’t need to consult with him if your treatment is equivocal - ie, use ADT or not…
Decipher becomes the ‘consultant’!
His videos are not public service announcements - they are well choreographed advertisements which are geared to draw eyeballs, consults and cash…
I’m not knocking his expertise ( double board certified oncologist)but his celebrity comes from his book ‘Invasion of the Prostate Snatchers’.
His niche, if you will, is to be contrarian and offer hope that things might not be that bad. But on the flip side, how many men who really needed their prostates removed, read his book (or simply saw the title!😵‍💫) and decided that their doctor was harming them? Probably too many to count. Best,
Phil

I am 5 weeks post op with Dr. Amling. I chose surgery because of my high decipher score as well. The Cath was not as bad as I feared. That 8 days went pretty fast. Recovery going well. Just had my first PSA and it was 0.1. Urinary issues exist but it seems to be improving a little each week. I manage well with Tena pads. I used depends at night for the first couple of weeks but found I did not need them. Dr. Amling said nerve sparing went really well and I am able to get an erection already. Have not been sexually active yet but am pleasantly surprised at the early response. Good luck to you.

I am extremely active before surgery and will continue that trend after as
long as humanly possible. I'm not going to worry about what could happen
down the line and take it all in stride.
