FESS Revision Surgery

Posted by tpajcrew @tpajcrew, Feb 20, 2023

I had FESS revision surgery two months ago, end of December 2022.
I had openings in all my sinus cavities and polyp removal. Two months later, I'm still dealing with off and on again inflammation in my nose. It's very uncomfortable at night especially, which is preventing me from getting sleep. I haven't slept much, which I know may be delaying my healing. My ENT says the sinuses look great but I have this congestion and just don't feel back to normal. I read various periods of time for healing, but what is the answer?

Hi. I can maybe help.

Is this your second FESS surgery?

Do you have allergies? And what are you currently doing for inflammatory control from your surgeon?

Mannnnn.... I have had two FESS surgeries, turbinate reduction, septum correction . My last surgery was November 17, 2026. Fluticisone, certirizine, Afrin, benadryl , Lawd. Ears that stop up often, mucus, blood clots ( dry and wet) Navage rinses....plus allergies allergies allergies. Prednisone. Antibiotics.
Emotionally drained.

I hear you...have had sinus issues since a bout of C. Diff in 2010. They got much worse with/after Covid. I feel like crap a lot of the time. Have had a balloon procedure and antrostomy, which didn't help much. I do a.m. and p.m. rinses with budesonide+ mupiricin + baby shampoo, use all kinds of sprays from ipratopium bromide to allergy sprays. Still have yellow mucus coming out pretty much every day after the rinses. I dust, have an air purifier, correct bedding, eat healthy...My ENT says sinuses look OK but mucus is pooling, not circulating. He put off but now thinks maybe a revision antrostomy will help the outflow. No guarantees and the downside is you're opening nasal passages up to more environmental allergens. I have dizziness, ear fullness, loss of hearing, tinnitus, too. Hard to parse what's sinus and what's post-Covid -- they overlap. Anyone have a similar experience?

A manually opened nasal/sinus cavity is naturally going to be more drying and alter the natural physiology. The majority of what is ever felt in the nose is driven off of the entire Trigeminal system with many parasympathetic/sympathetic drivers.
Only 0.001% of anyone entering an ENT clinic would ever “need” benefit from a surgery.
ENTs have been poorly educated since the 80’s. It has been slowly catching up to their community through more studies that prove such.
Soon enough these surgeries will be highly discouraged and more patients will learn not to fall into the ENT sales pitch.

@nrd1 Thanks! Your input gives me added pause. It's not that my ENT is rushing to operate -- in fact, he's held back on the revision for well over a year. I think at this point he doesn't know what else to do. Cutters cut. Think I'll put this off and keep looking for other/better answers...I will now google the "entire trigeminal system"!!!

@shashig I had sinus cancer treated with radiation therapy which destroys cilia among other things. After adding xylitol to my budesonide rinse the mucus cleared out better.

@shashig morning. I’m set up for a Maxillary Antrostomy on Oct 7th… my left maxillary has had and may still have the bacteria Pseudomonas . Ive had live infections both in the left maxillary and in my lungs . Drips of the bacteria drip down your throat and into your lungs . Right now , the cultures and lab reports are showing moderate Pseudomonas colonized . Which means there is no live infection in both the sinus nor the lungs so no need to treat . I was on IV antibiotics for 2 sessions last year 14 days and this past Feb for. 21 days . My ENT Dr thinks he will be able to eradicate the Pseudonomas with the surgery .
My Pulmonologist is doing a chest CT scan to look at nodules in my lungs as one has grown by 15% in 6 weeks. So that will be done soon.
I guess I didn’t realize that people walk around with bacteria all the time . I’m 85 years old and this is a first for me !
I did have COVID back at end of 2019 Nov and Dec.. right before Covid was named Covid in Feb 2020 . Although it’s not official on the hospital records as such . However my contingent of Doctors now think that’s what I had that caused heart and lung damage ! I also have Tinnitus in the left ear . Seems everything is on the left !
Al, the best, take good care and sincerely hope you can get yours under control. Kay B.

@jacbapple Thanks...yup, I do that. Make my own saline combo of salt + xylitol 1:1 instead of salt + baking soda 1:1. I buy the xylitol from Amazon (see link). Among other benefits, it's the cheaper way to go instead of NeilMed or other pre-packaged branded products. I mean... why pay for all that packaging when you can DIY?

@kaybast You too, Kay. Yes, we all have stuff living in our nasal pasages. It's when the population changes from "scant" on the test to a bigger presence...I typically add just a few drops of extra ingredients to my nasal rinses -- herbal tinctures said to kill bacteria/fungus; i.e., barberry, crytolepsis...figure it's topical, it can't really hurt.
