Anyone reinstate Effexor (Venlafaxine) after 9 weeks off to taper more

I was on Effexor (Venlafaxine) ER in January 2025, with my highest dose being 225 mg. I didnāt feel like I was getting much improvement with Venlafaxine, so I eventually tapered down gradually and slowly to 75 mg. Since the ER is a capsule, I asked my doctor to switch me to the IR so I could taper more even slower. Per her taper recommendation, I stayed at 25 mg for about 2 months, then went down to 18.75 mg for about 4 weeks and 12.5 mg for another 4 weeks and went down to 6.25 mg for another 4 weeks. Since I couldnāt cut the dose any lower, she recommended taking 6.25 mg every other day for about 2 weeks and then stopping. I stopped completely on June 4th.
Iām now 9 weeks off, and my doctor believes Iām still experiencing withdrawal symptoms. Since June 10th, Iāve felt keyed up much of the time, with adrenaline rushes, nervousness, rapid heart rate, and waves of anxiety. Iām also really sleepy, sluggish, and feeling wiped out. Even after sleeping 7ā8 hours, I still wake up feeling like I havenāt slept. The symptoms continue from morning through the evening and are really affecting my quality of life. My doctor also prescribed Prozac to help with the withdrawal symptoms, but it didnāt help, so I stopped taking it. I take 10mg of propranolol in the morning to help with some of the anxiety symptoms.
I met with her last week and she said she did some research and also talked with some of her colleagues about my symptoms and they recommended that I reinstate at a very low dose and then taper much more slowly. One option she gave me was to restart with a 75 mg Venlafaxine XR capsule and count the beads, taking 10 beads (about 5 mg) to start. The plan would then be to gradually reduce the number of beads: 10 ā 8 ā 6 ā 4 ā 3 ā 2 ā 1 ā 0, staying at each dose for 2ā4 weeks or longer if needed. She also found a compounding pharmacy that can make liquid Venlafaxine IR, which would allow for even smaller dose reductions, but I am really hesitant to restart Venlafaxine. I worked hard to get off of it, and it wasnāt really helping the anxiety symptoms when I was taking it. I also donāt want to go through withdrawal again after I eventually stop.
Has anyone else been in this situation? Were you still having significant withdrawal symptoms 8ā10 weeks after stopping Effexor (Venlafaxine)? Did you decide to reinstate and taper more slowly? If you did, did it help? If you decided not to reinstate, are you still having withdrawal symptoms, and have they started to improve with more time? What did you do that helped?

@depressedbutnotdead It really is worrying how meds can affect people. Very interesting reading what happened to you. Its great that you took control yourself....so many just go along with the Drs opinion, often ending tragically.

She wrote me back today to say she believes Iām dealing with ongoing venlafaxine discontinuation symptoms, and some return/worsening of the anxiety and mood symptoms that venlafaxine had been treating š¢š¢

I did not have any of what you had while on Venlafaxine for 19 months, but since I stopped taking it 10 weeks ago, I am now dealing with uncontrollable anger, irritability, sadness, anxiety, feeling wiped out, and short tempered. I refuse to restart it via the hyperbolic method of counting beads and tapering slowly at 5mg. My doctor respects this and isnāt pushing me to try it. I messaged her today and she believes I am still dealing with ongoing venlafaxine discontinuation symptoms, and some return/worsening of anxiety and mood symptoms that venlafaxine had been treating. I have an appointment with her in 2 weeks and she said if I want, I can try a different antidepressant with what she describes as a āmuch gentler discontinuation profile,ā starting at a very low dose and making slow adjustments.
My previous doctor prescribed Lamotrigine when I was on 75mg of Venlafaxine but I started having daily rapid heart beats and it became bothersome so I stopped taking it. Iām glad things are pretty settled now for you š¤
@wynnaa I think my reply went through with a lot of other stuff ahead of it? ? Yes I was on a slow release, as its called here. There is no lower dose available in Australia than 50 so I went on to a 40 capsule and then back to splitting the 50 to get onto 25 daily then capsules of 10. Yesterday I decided to take 2 x 10 to stay on 20 daily. I opened a capsule yesterday, its just powder so no counting beads here. Your capsules are largeish beads? I might end up cutting the 50 til it cant be cut any smaller...wont be getting an even dose each day then. Not sure if Compounding Pharmacy can do a 5, why not? I feel like Im all over the place changing dosage often, not a good idea really šhoping I can feel normal!!
@cas2001 In the U.S., Venlafaxine comes in an extended release (ER) capsule, which contains small beads. The lowest ER dose available is 37.5 mg. There is also an immediate release (IR) version that comes in tablet form, with a 25 mg tablet being available.
When I reached 37.5 mg, I switched from the extended release capsule to the immediate release tablet so that I could make smaller dose reductions. I took 25 mg for about two months, then reduced it to 18.75 mg and stayed at that dose for six weeks. I then reduced it to 12.5 mg for four weeks, followed by 6.25 mg for four weeks. After that, I took 6.25 mg every other day for two weeks and then stopped completely.
Looking back, I think I may have tapered too quickly once I reached 6.25 mg but i followed the instructions of my doctor. I have a follow up appointment with my doctor soon, and Iām considering asking about restarting at a very low dose using a liquid formulation prepared by a compounding pharmacy. My thought is that this would allow me to stay at each dose longer and make much smaller reductions as I taper off very slowly. Itās been 10 weeks since I discontinued Venlafaxine and I am not feeling well.

It sounds like you have a great doctor, but the experts recommend no more than a 10% decrease over time and not skipping doses. At the risk of sounding like a broken record, I recommend (to you and your doctor) The Maudsley Deprescribing Guidelines.

@wynnaa I wonder if we're going to be left with some sort of withdrawal for a time, no matter how slowly we taper. I wish we had the beads over here to taper with although youve done it very sensibly but still not feeling good. When I came off of Mirtazapine I felt so good that I was finished, tapering slowly myself. Ive never heard anyone post that they felt great once they finished this particular drug ā¹ļø. I think you'd do well with the liquid compounded script, tapering more slowly. Id give it a go! Hope it works really well for you. Keep posting .
