Failed back surgery?
This is my latest mri report. I had fusion, laminectomy and decompression at L5 S1 due to congenital spondylolthesis and bilateral pars defects. Lots of pain and not sure what to do? All other lumbar levels were normal so I did not include them here.
At L4-L5, facet joint hypertrophy. No spinal canal narrowing. Mild bilateral neuroforaminal narrowing
At L5-S1, grade 2 anterolisthesis. Disc uncovering. Disc bulge. Spinal canal is decompressed by laminectomy. Facet arthrosis. Moderate bilateral neuroforaminal narrowing. Evaluation degraded by susceptibility artifact from hardware.
Abdomen/Retroperitoneum: Multiple cysts are noted in both kidneys.
Fusion hardware is noted across the right SI joint. Joint space narrowing of bilateral SI joints.
IMPRESSION:
Posterior instrumented fusion and laminectomy at L5-S1. Grade 2 anterolisthesis L5-S1. Facet arthrosis with moderate bilateral neuroforaminal narrowing at this level. No change since prior exam.
Facet joint hypertrophy at L4-L5 with mild bilateral neuroforaminal narrowing. No change as prior exam.

How long ago was the surgery?

Lumbar fusion was in February 2022, right SI joint fixation was done in November 2005.
@jeaniem Wouldn't it be great if the radiologist would give you some idea of what all the MRI gibberish means in plain English. And what some possible treatments might be? I know, hopefully one of your pin docs will tell you. You may have to wait a while to get an appointment.
All reads good to me.
I’d say it was a successful surgery.
You need to sit with your doctor , with your report in hand and have him/her explain it to you for clarification in layman’s terms.

1 ReactionPain management Dr says it is a failed back surgery. Suggests spinal cord stimulator or more steroid injections. RFA did not help.
