What kind of recovery can expected after HoLEP at age of 72
I am a 72 year old male, retired I am planning to have HoLEP surgery at May Clinics in two weeks . following history and background, I
I have been taking medication for the last 12years, with IPSS-14 to 19. I am taking Finesteride (5mg) and Tamsulosion ( two 4mg) daily. I believe my symptoms are moderate. The number of urinations per day on average 10 to 12. The average number of wake ups in night are at least two. three times has become more common lately. Often it takes 10-15 min to void the bladder in multiple streams. However PVR per Urologist's exam is fairly low ( 63 to 6 CC). Past biopsy, MRI and the PSA blood test(2.5) has ruled out cancer . The MRI report on 5/26/22 was 95 gram of prostate and Pir Rad 3 . Take MRI for prostate size every year, the size of Prostate is measured to 100 and to 110 in 2026
I am scheduled for the HoLEP surgery in two weeks. Wonder what can I expct in terms of post-operative phase in terms of pain, discomfort and recovery ?

I'm 80 and have a couple 4+3 areas (see my profile for more deets) but I am considering Ho-lep to reduce prostate size (114cc). Also considering focal therapies aimed at the area where the PET scan shows my cancer. MRI does not show specific lesions. IPSS scores are pretty low, but at my age, the likelihood is that things will eventually get worse.
With my large prostate, some other PCa treatments are precluded, so Ho-Lep could reduce my prostate size enough to allow other treatments, if they're needed.
I had holep procedure 3 months ago. Post surgery my perineum area had a painful burning sensation. Making it difficult to sit. I still have it today and started spot bleeding again. Dr. says I am taking a long time to heal. Anyone else have a similar experience?
Hello apex1111….I am 74 and HOLEP 4 years ago at the Mayo Clinic in Jacksonville. When you have the procedure, you’ll wake up with a Folley Catheter in place. You’ll spend the remainder of the day and that night doing nothing but laying in your bed. It isn’t real comfortable of course, but it’s only short term. You’ll likely go see the surgeon the following morning, they removed the Folley for me at that time. A little later, after you have consumed some water, they’ll measure how well you urinate. The measurement includes not only how much you urinate, but how much is left in your bladder! Also, they produced a bell curve that shows how well you urinate. You’re going to be amazed…..I was. When they give you the green light to go home, you’ll resume normal living but taking it easy for a few days. You’ll likely notice that you’ll pass some small pieces of tissue for a week or two, but it eventually stops. You will be very pleased at well you can urinate! If you’re sexually active, you can resume after six weeks. This is where it gets different. You no longer ejaculate during climax, but it still feels wonderful! I am very happy about my HOLEP Procedure, I hope you are as well!

1 Reaction@ctsenior46 I don't think you should have any bleeding at this point from the HoLEP. You could have bladder stones, stricture or something else going on? I also had the pain sitting after my procedure. Right at the 3 month point it went away, so be patient. The prostate and rectum are innervated by the same nerve and I think HoLEP can injure/irritate it.
I had the surgery a few years ago. In the days after I had no pain, just discomfort for awhile. But then I got a bladder infection and had to be hospitalized to clear that out. Otherwise the surgery was successful and I am back to normal now.
@medinfo1 thank you very much for the suggestion "Had Holep completed 1 year + 8 months ago with Dr Dora at Mayo Jax. Surgery included a Botox injection (ask about it). ". Dir.Dora is my urologist a well. SO I will ask him .I do not have any issue of incontinence or urgeny . Just weak flow . (about 48 fows with avg max 11:81, average 5.6 and average volume 199 mil)
