After Holep leakage
I’m 51 years old, for last few years my PSA was around 17-21 and prostate was big ( about 90 cc) - several biopsy showed no cancer- doctors in Kaiser San Fransisco area recommended Holep which I decided to go with and had surgery on January 6, removed catheter the following day , for a week after I had uncontrolled leakage and dripping with blood and tissues coming out with urine. After this week blood stopped and urine was clear but annoying dripping continued . Six weeks after surgery suddenly I had a flood of blood in the urine with some tissues or clots lasted for 24 hours then back to clear urine with few drops of blood either in the beginning or at the end of urination. Surgeon said blood in the union can be on and off for three months and leakage could take up to one year . Anyone have similar experience. While tankful, Having to wear diaper in my age keeps me of bothering me.

Sorry to hear this. Luckily I had almost no bleeding or leakage.
I had frequent urination issues afterward, as I had also before the holep. Surgeon suggested pelvic floor physical therapy and it proved to be very helpful.
Has your dr suggested this? Insurance covers it with dr. RX. Just FYI I went to theorginwaydotcom, they have offices all over.

1 ReactionI had HoLEP in December 2024. I went through 6-7 diapers a day for about 4 months while religiously doing my kegel exercises. After about 8 months I am down to a liner in my underwear. It has now been 15 months since HoLEP and I am still leaking just a tiny bit. I just underwent TURP surgery 3 days ago due to recurring UTI’s after the HoLEP. My urologist said this TURP procedure was successful and the UTI’s and leaking should go away. I don’t mind the small amount of leaking, but the UTI’s need to stop. I had 11 UTI’s in the last 15 months. Miserable not knowing when another UTI is coming.

2 ReactionsI am eleven months after holep and continue to leak. Still doing kegels. have a urodynamic test coming up. Outlook is not good. I am 68, good health, large prostate. No leakage prior to holep. My sphincter is still weak. Not sure this holep thing is all it is supposed to be. Talking about an artificial urinary sphincter now. I need to see Mayo clinic. The Dr at Penn State Hershey medical that did the holep procedure has not been helpful in the least.

2 Reactions@elcorbs Update from the TURP procedure: The catheter was removed yesterday morning. Before removing the catheter the nurse put fluid into my bladder to ensure I could urinate on my own. I did and went straight home wearing a diaper just in case of leakage. I got home and drank water all day. I flowed like a teenager. No joke. Just a small amount of blood and burning while urinating. Took meds for that and I slept so well last night. Woke up 2 times to urinate and again it flowed great. Now when it stops flowing, I know I am empty. Praying this eliminates the recurring UTI’s. I do believe my doctor got this right.

4 ReactionsI live in freeport, IL. Doctors around here don't seem to know what I'm going through. I have a issue with peeing at night. I have to get up every hour at night to pee. Something I can't make it make it to the bathroom it just leaks. I pee every hour no joke through the night. Doctors here don't seem to know why. Can any one help me. Please. Still trying. The Dr. put me on two scripts a month ago. One to relax the bladder and one to tighten the sphincter. Had a urodynamic test a couple of months ago. Still a long way from a normal quality of life but better and more manageable from six months to a year ago. When I went to Johns Hopkins for another opinion, they told me flat out the Holep caused incontinence. I continue to slowly get better each month but I am having doubts I will get to a normal life again without the artificial sphincter surgery that I would rather not do. I continue the kegels every day. Nighttime is somewhat of a relief with little to no leakage. During the day the more physically active I am the more leakage there is. Several pads per day. I think time is running out.
I am 18 months since HoLEP and continue to leak. I stopped pelvic floor exercises a while back and just started them again to hopefully bring this to an end. I use only 1 guard per day, so not really bad. I don’t need a guard at night.
I also had TURP on 3 Mar 26 and now flow great. The issue with my recurring UTI’s still remain. After surgery I went 45 days without a UTI and now 2 or 3 days after 10 days of antibiotics the UTI comes right back. Currently taking Bactrim 2 times daily for 20 days then starting Nitrofurantoin for 90 days. All would be great if I could get rid of the UTI’s.
Had Holep on June 19, 2026, It was a painfree, uneventful procedure. My 1st week post Holep has arrived.
Day 1-4 usual, expected loss of bladder control - 1 Level 3 Depend per day except day 4 when 2 pads were required.
Day 5 - 6 had temporary blockage that eventually resolved itself - so almost no leakage during this time.
PAIN: hardly any except initial stinging pain passing urine; this has receded to a very dull sting by day 6/7. Took 2 panadols 2 hours post Holep - those were unnecessary as I had zero discomfort or pain!
URINE: still totally clear despite being back on anti-cogulant 12 hours ago, after 11 day off it.
No running for 4 weeks; the earliest I can return to running will be July 19.
