What should we take with us if we have to go to Emergency Room?
Photo ID, Medicare and other Insurance cards?
Do they need to be originals, or can they be copies (in case we lose them in the hospital)?

Thanks, everyone! 85 year-old hubby had to go to ER today, and I want to add to the list what the Emergency Medical Squad asked for when I called 911 and they came to pick him up: his social security number.
So, you probably know yours -- but do you know your spouse's?

What brand of disposable underwear does he like? My husband is just transitioning into disposables.
For years I've made my own medical summary, I put in a computer document and print ONLY ONE PAGE front/back.....you can adjust font size, bold/underline important items, and carry extra copies.....doctors and nurses always want a copy (they love it and you won't forget something important when under stress, medical personnel always ask same questions, and you'll always forget an item)
Patient name (nickname), address, phone, date of birth, single/married/spouse's name,
Emergency contact- name, phone, relationship,
Medical insurance plan #, member # (self or policy holder's name), employer/occupation/address/phone
Current medications/over counter/supplements,
Allergies- medications/foods/substances,
Vaccinations (which and when),
List with dates of medical issues - current, last 5, brief one or two words, dates List of hospital admissions, diagnoses,
List of chronic conditions with inception dates
List of surgeries, dates, implants
Brief Family history
Parents, Siblings, grandparents (alive, current age or DOD cause of death, important medical conditions)
Finished (date)/Pending Procedures that need to be done ......dexascan, vaccinations, mammogram, colonoscopy, etc

He likes the Depends, but during the summer they can be kind of hot and he can get a rash. He has tried a couple types of Dry Direct samples available at Parentgiving online and liked them very much—so I am about to order some of them too.

1 ReactionIn your documents, have a list of your medical providers. Name, address, phone and fax numbers, specialty. If you are a dialysis patient, make sure you have a current copy of your transfer orders. Those are usually provided every 3 months to you. I keep a master list that includes my POLST and it goes in my bag with me everywhere. Ginger

1 ReactionI take copies but also have a to go bag ready at all times. Under clothes, shorts, toothpaste and toothbrush, snacks and water!!! Phone charger and a book!

1 ReactionOne time, I made an appointment online at local urgent care. A nurse promptly called me back, asked a few questions and told us to go straight to ER and they’d call so ER would be expecting us. It saved us from having to wait at ER and H was whisked right in when he presented his ID and given attention & a room instead of waiting endlessly in waiting room or on gurney in hallway. He was promptly given room with a bed too.
If you have time and the system allows you to, trying to make an appointment to urgent care or ER may be helpful sp they know to expect your arrival.
H really liked having his phone and backup phone charger & cable.

2 ReactionsInsurance cards, photo ID, know the last four digits of your social security number. List of all your current medications , physicians names, Copies of your Medical Directives, and a Power of Attorney.
Just stating you are the patient’s spouse no longer gives you the right to make medical decisions for the patient if they cannot speak for themself,
Other non medical personal items can be brought to the hospital later should they be admitted.
