Scared of pain during L4 & L5 nerve block: Please advise

Posted by marnie51 @marnie51, Aug 30, 2025

I’m 74, female with degenerative disc disease with some spinal stenosis. I have severe arthritis in my lumbar L4 and L5, causing debilitating pain. I’m told I need a nerve block. If that doesn’t work a procedure called an ablation. My problem is pain. I don’t tolerate pain very well and I am scared to death of experiencing any sort of pain during the block. Please advise on anything that might help me.

After the MRI's, my doctor referred me to physical therapy twice a week for 6 weeks. St the same time pain mgmt first gave me a shot to deaden the nerves. That was good for 2 days and all the pain came back. Next, was a joint block shot. It did not help at all. So now I'm scheduled for disks to be rebuilt on L-2, L-3,
L-4, L-5 and S-1. It's been a long road that I hope is almost over.
It was a process of elimination to see what was causing the pain. If yours is from arthritis, I would ask him about a joint block.
It says that if the nerve block removes 50% of the pain, then you are a good candidate for ablation. I'm 70 yrs old. I hope you have someone to be your advocate if necessary.

@yesibeleive are you saying the nerve block didn't work for you? My husband is in a similar situation as you..physical therapy, steroid injections and pain meds. He's at the highest pain meds and still in excruciating pain. Surgeon says the surgery is so complex, not to contact him until he gets to the point he cant get out of bed or loses bladder function.

I had two of these and a pain stimulator and neither one of these worked.

@niecie66 I was thinking of the pain stimulator for my husband since, PT, steroid shots and pain meds not working. Were you able to find any relief?

Ask for something to relax you before the procedure like Ativan or something, it helps a little for the nervousness.

@vicinsoflo oh, not no, but, **** NO!
I had my MRI's..went to PT...received 2 types of shots. I that order. Then went to surgery on L2,L3,L4,L5,S1. I got no pain meds til after surgery.
I go to Dr Praveen Reddy/neurosurgeon in Conroe, tx
I'm pretty sure it's time to see another doctor. It doesn't sound right.

@anglndskys everyone is different. I am 71 yrs old. Mine was L2,L3,L4,L5,S1 + scholioses. By 6 months I was walking w/o a walker. But, always causiously. I'm still uncomfortable most the time but not really in pain. They had me wear a vest that had to be charged. They said it was like a tens unit. Also..they wanted me to do aqua therapy to strengthen my core but, couldn't find one that ins would cover. I hope you are going to physical therapy.

@vicinsoflo I'm so sorry he's having to go through this. It's so hard when you (your husband) is the only one feeling the pain.
Please keep me posted.

@vicinsoflo tens units are non effective in our position. Ot/pt, injection for nerve endings, injection for joint pain (process of elimination to see what is causing the pain), surgery.

@yesibeleive thank you so much for your continued responses. I'm floundering trying to find a way to help him, as he seems to be giving up because of the severe pain. He doesn't even think he could make it to a new Dr appointment. Can the pain be that extreme? That's why Ive taken over doing the research. He's had several caudles/epidurals that didn't have no effect. Could it be the Dr isn't shooting it in the right nerves? They are xray guided injections, too.
