Not Good News after prostate biospy when MRI didn't look too bad
Last month I had PSA of 5.23 when a few months earlier it was 3.2. Then they scheduled me for MRI of prostate. Did another PSA and it was down to 4.16, but still wanted the MRI. Report is below, doesn't look good PI-RADS 5. At one point they say in report Lesions (PI-RADS 3 or higher). If I understand it, it hasn't spread. Wish I could get a plan with doctor!

IMPRESSION:
1. The prostate gland measures 5.7 x 5.0 x 4.9 cm with volume of 68.75 cc. PSA density is 0.06 NG/mL/CC. 2. Lesion # 1: PI-
RADS 5 lesion in the left posterior peripheral zone extending from the base to the apex measures 5.83 cc. No frank extracapsular
extension. 3. No pelvic lymphadenopathy.
PI-RADS Category 5: Very high (clinically significant prostate cancer is highly likely to be present)
Really doesn't look to bad, one spot that hasn't spread!
Then Bad Update 2/10/2026
Well got biopsy yesterday and results today, doctor hasn't called, just sent biopsy results to MyChart.
The MRI showed only one Lesion like shown above. Had biopsy done yesterday, they did 3 from the Lesion and 6 from each side of prostate. I wondered why they did more biopsy that were outside the lesion, but didn't ask. Got report today- not good. The lesion look better than areas where MRI saw nothing. They took 15 samples total.

Another thread I posted in a person said "You have a Gleason 4+3 7 BUT you have large cribriform and doctors a UCSF say that puts a 5 in your Gleason score." I believe he picked this up from the biopsy report. I don't know what a cribriform even is, it's not mention in report. From googling around it can only be determined by sieve-like or "Swiss cheese" appearance under a microscope and I don't see that in report? But this is all new to me. Doctors haven't talked to me yet, who knows when they will call or make appointment, took long time to get MRI and even longer to get the biopsy done. Sure were fast getting results, they said 7 - 10 days and they gave them to me the next day. Kind of wish they didn't give me results prior to talking with me.
My first thought is just get the thing cut out, not sure how that is done, as seems they got to leave something in there for urine to flow threw. So they couldn't take 100 percent of prostate out. Then I read about nerve sparing or not and not sure what that means. No doctors have discussed this with me yet. Seems if they take it out there shouldn't be any prostate cancer left? But then I read where people get it out and still have a PSA level, so like I said earlier, they must leave some in there, even when they call it total. Had to drive 150 miles to get MRI and biopsy They could have done that in Topeka, but KUMC is ranked as number 50 in top of prostate treatment so I went there Topeka doesn't have a Proton device, that would be back up to KUMC 150 miles RT. One of those radiations therapy is only a few days, not 30 some days. They do have SBRT radiation in Topeka, but I know of someone who had SBRT or maybe it was IMRT and it screwed up several other organs around the prostate, like bladder, kidneys and intestines.
Then some tell me I am lucky to have them all in grade group 2 or 3. But seems like I had a lot of them (12 of the 15) . So I would guess if they did 25 biopsy I could have had more grade group 2 or 3.
All confusing and stressful, other that this I am 78 years old healthy as a horse- no other issues and very active. Loss of what to do and all the different radiation types, that why just getting the pesky thing cut out of there, but seems they still leave some in.

Well they called and said overbooked and moved me to June 17th. I think what that really means is someone more urgent took my place, really tired of these doctors. I thought I posted this once, but maybe I didn't hit POST? This whole process is just about too much. Dealing with the physical and emotional issues are bad enough, then all the scheduling and auto attendants when making a call is just too much.

That’s one thing I like about the whole patient portal concept. I rarely have anything urgent to discuss, so I can just pop in there, leave a note, and it’s usually answered same-day, or a least by the next business day.
Before, when it was all phone calls, I’d call my doc, get voicemail with the instruction to leave a message and someone would respond within 72 hours…and then they wouldn’t.
Maybe you can check back and ask to be put on a cancellation list so you can get in a little sooner?

2 ReactionsI did finely decide to weigh an overnight depends for the last couple nights. I just use bathroom scales, they weigh nothing when put on, weighed .2 pounds this morning. That comes to 3.2 oz by my math. I had got up 4 times with strong urge and had a good strong pee each time. Must just leak through the night to get that 3.2 oz. I go to bed around midnight and get up around 8:30 -9:00. Also don't drink anything after 7 or 7:30 PM. Don't know where it all comes from, but good strong stream. The prior night they weighed .4 pounds in the morning.
Wish I could get that urge and pee like that during the day. During the day it's just dripping and stress incontinence. If I been sitting visiting for an hour or so or laying on couch watching TV I might have slight urge and a small stream. But not when moving around.
I do about 20 kegels and hold each 1 for 10 seconds, relax and do another one for 10 seconds. Then do about 100 that I just hold 1 second each.

How many times a day you do those Kegel sessions ?
Also, bathroom scale is really not good for measuring small amounts of anything, it is far from precise : (((. If you just move the scale little bit and than measure the same pad it will most probably give you a different result. BUT, at least you got some idea about the amount, I guess. Digital small scale is only $16 on Amazon.
3.2 oz are equal to about 95 ml , and that is moderate leak and almost mild. That is encouraging and will continue to improve over time - you are doing great this early into recovery and with having TWO extensive surgeries.

2 ReactionsI do 2 sets of 10 kegals, holding each kegal 10 seconds.
Then I just do 2 sets of kegals holding 1 second. Do 50 of these 1 second ones in each of 2 sets.
Might look around for small digital scale, would be nice to see progress. Not sure I would want to measure the one throughout the day as sometimes I let them get pretty soaked, depends on what I am doing or where I plan on going. It's not the cost, just haven't seen any.
I put 3.2 oz water in measuring cup, really looked like a lot. As much As I pee at night with strong stream (4times) and no drinking after 730pm, seem a lot, but I think it was worse, just never measured.

Not sure you saw the messages I posted about doing Keagles. A person at one of the meetings I was in said he did 100 of them every day. Two other people in the meeting had been with physical therapist do that specialize in incontinence. They were told by the physical therapist that doing that many kagles was not a good idea since it weakened the muscles since they became tired after doing it so many times. Quality over quantity is better.
Check with a physical therapist, Maybe they have different opinions.

1 ReactionThat brings up a good question as to what is a complete Kegel?? Holding for a period of time or one quick one? It seems from a quick search I get this:
A single repetition is defined as contracting these muscles, holding the squeeze for 3 to 10 seconds, and fully relaxing for the same duration. Also, found where it said to 10 Kegels in the morning and 10 at night.
However, a Mayo site said- Aim for at least three sets a day. Work up to doing 10 to 15 Kegels in each set (but they said to hold for only 3 seconds). Another site said hold 3 - 5 seconds. Think I like the hold 3 - 5 seconds better than what I been doing.
Think I will stop those 1 second Kegel, didn't find them any place.
I think this is one of those frustrating things medical advice - it differs from specialist to specialist.
I’ve had a nurse inside the same practice tell me I was leaking way too much, and another one at the same appointment say I was right where I should be for recovery. 🤷♂️
I’ve seen “official” exercise advice that simply covered starting with quick-hold 1-second kegels and working up to 10-second kegels, others saying it’s important to do both short and long-hold kegels, as that firms up different parts of the muscles…some say never go over 50 in a day…or 60 in a day…or 80 in a day. All do agree on burning out the muscle at some point, so I never went over 60 in a day. I don’t do that many now.
I think it comes down to picking one or two experts (a trusted nurse at the practice and my PT specialist are who I settled on) and follow them. I think there’s more than one way to skin a cat…you just can’t skin it all the ways at once.
Pads - not only do I weigh each one (and yes, some can get pretty heavy), but I’ve taken to computing a weekly average. I’ve found that is much more helpful in determining improvement over trying to figure that based on daily weigh-ins, simply because I could have one day where I leaked out 712ml and another day at 284ml, and a third day at 555ml…all in the same week and drinking the same amount of liquids over the same period.
Am I better? Worse? Staying the same? Tracking a weekly average shows that I’m definitely improving week-to-week, and it’s easier to see that compared to my scattered dailies.

Yes - one should keep a log ! Total of urine in ml in every 24 hours period and total in a week.
Kegels - my husband did hundreds a day, he did sets every hour whatever he was doing , never got any symptoms of "tiredness".
He did it while working , while walking, while standing in grocery lane, while talking shower, while sitting on a toilet - I really do not see how those sphincters can get tired, but I guess everybody is different. He also did once a day full pelvic floor exercise program that he got from YouTube and books.
He regained full continence 4 and a half months.

Yes bathroom scales not so good, weighed today and it was flashing .2 then .4 and never did stop. Then I moved to one side and it was .4 and on the other side .2.
Wife said keep away from her kitchen scales with that, so went to Walmart and bought a cheap $15.00 kitchen scale. It doesn't use ML, it only measures in OZ and G which I believe is Grams. That is okay as I understand ounces better that other measurements.
I am just going to measure the overnight one, don't think I have what it takes to keep records of all pads?
Wonder why I see all these post in MLs? Wouldn't think any kitchen scales would use ML, surprised the one I bought had Grams. I could convert if wanted to, easiest way to do in head is multiply by 30, even though is really 29.57 (can't do 29.57 in my head).
