Gluteal tendinopathy - endless pain - losing my mind

Posted by emarks @emarks, Nov 29, 2025

Has anyone else dealt with this:
I'm now up to 5 months of daily pain, the the last two weeks have been the worst. I'm totally losing my mind.
First diagnosis was L5/S1, with disc pressing on nerve. I had cortisone shots, then an epidural. Not much relief, although with time the L5/S1 pain seems to have subsided. I've been doing physical therapy for almost the entire time.
For the past two months, increasing pain in left buttock and often sharp pain in left hip area. Diagnosed with piriformis syndrome, new MRI shows gluteal tendinopathy. I got two cortisone shots for that 6 days ago.
The hip area pain just keeps getting worse. I've had to cancel many plans and limit my activities. It's very depressing and is driving me crazy. Just wondering if anyone else is dealing with this and if they have any suggestions for both physical and mental health. Thank you.

I am like you in that I used to keep up with friends and social activities. My husband and I used to go to national parks and do small hikes. I loved it and miss it. It's weird that mine comes and goes and moves to different areas. My Gluteal Tendinopathy diagnosis came after 2 years of thinking it was just bursitis. Collagen depletion in tendons is real and what can be done. Softwave or Shockwave seems to stimulate and regenerate the cells in the areas you need help. I notice my upper outside glutes will all of a sudden just pull and spasm when doing light yardwork or housework. It will pull all along lower back and spasm so bad on just certain movements, I have to stay in bed with ice and heat and NSAIDs. It takes a good week of resting and slow movements to feel better. It stays tender for a long time. I had also just started up my Repatha again, so now I wonder if it was that. I also found out my adrenals are fatigued and does that have issues with muscles. My Ferritin was borderline low. Would taking Estrogen help? So many questions and no answers. I was doing the main glute loading exercises like Bridge, adductor leg lifts, sitting clamshells (not laying on side). I found a Facebook group under Gluteal Tendonopathy groups with a Physical Therapist as the moderator that listens to our issues. I do feel building glutes back up helps and maybe doing Softwave. Some collagen support is recommended but I am on the fence on those.

The "best" collagen seems to be marine collagen, which has both type I and III collagen.
Might want to try that.

@lkayhoff I have the same questions and I get no answers. I'm in pain most of the time. I've attempted to make an appointment at Mayo and was refused three times. It just keeps getting worse. I gave up my cane and went to a walker about a year ago. Next will be a wheelchair. A week ago I had a PRP injection as a last resort and so far have noticed no improvement. I first had pain after I fell in the army in 1975. They gave me a 0% disability rating. I gave up on trying to get compensation. If you read "The Women" you will understand this better. From the time I fell, my left hip has been higher than the right. I doubt any of this will ever be favorably resolved. Meanwhile, I'll just keep going to physical therapy like I've been told for decades.

It can take weeks (up to 6 or 8) to feel any improvement from PRP. Also, how many cc's of blood were removed? You should have 60cc's removed and doubly - not singly - spun by centrifuge to recover a sufficient number of platelets.
Best to you.

I understand completely. I am in so much pain. I can't go anywhere or do anything because of the pain in my right buttock. It hurts to sit . 2 weeks ago I feel and broke my femur . So now I can't walk for 8 weeks. I feel like I'm losing my mind. I hope you will get well. I pray that helps

I know all about pain so bad you are losing your mind. I had a bad fall in 2024 , the 3rd after a bad accident in 2017. I've finally gotten an appt in New York City at HSS for evaluation and complete overhaul because I've been "mis diagnoised and mishandled" by doctors in the city and surrounds tri state area. You may want to consider HSS.

NJ and NY have many good Drs. I've from Ohio and going to HSS in NYC. some others have seen a Dr Lall in NJ.

Hi all

I have too many things wrong with my gluten and my lower back. I did have a shot to break up my calcified tendon ( for those that do not understand it was 'frozen' it did not want to move along with bursitis., piriformis syndrome, thoughts that it was bone cancer in my hips, and muscle spasms so hard in my upper legs I could not walk.
What has worked for me: myofascial release, massage, cranial sacral massage (a real practioner not a pt or mt that says they know it) exercise, and a therapy pool (or bathtub). I aggressively fight the cause of pain. I have used the Shockwave treatment for other issues and it has worked. I hope that everyone wins their battles with pain.

@jfn I was just diagnosed with the same thing. Along with L5 radiculopathy and a baker cyst behind my knee.
Thinking about shockwave therapy which area did it work on for you?

Hi @tymlos666
I also do cranial sacral therapy with a real practioner, nor a pt or massage therapist who does CST. Big difference. Myofascial release works too. I did have a cortisone shot for my calcified tendons related to my glutes. I just love being a living fossil. I tend to be proactive with injuries, or they will haunt you later.

Have you been evaluated for a labral tear? I was misdiagnosed for almost a year. Need to see a doc who specializes in hips. Someone recommended HSS - I agree.

I have been to 9 doctors including a large teaching hospital. I have had at least 24 injections over the last 4 years. Nothing. Couldn't even tell I had them except for the discomfort at the injection site. A week ago today I had a platelet rich plasma injection. No sign of any improvement. My doctor told me several months ago that I just needed to live with it. If I have to spend the rest of my life in pain I hope I don't live much longer. Hydrocodone doesn't even help much. I've been doing physical therapy for 5 years and I just keep getting worse. All my friends are out enjoying retirement going to concerts and on trips and cruises and I can barely get to the car to go to the grocery store. The social groups i was in volved with stopped inviting me (I was told straight out that it was because I can't keep up). Sorry for venting-just feeling sorry for myself.

@bunstuffer here are a few ideas other than find a good pain doctor: try meditation, get outside & watch the birds, read: How to live well with chronic pain; listen to podcast bendybodies.com by Linda Bluestein Mayo Clinic trained doctor. Did you have an MRI done? Not knowing details I would get a second opinion on your hip. I feel sorry for myself, too. In May, our BIG coon hound ran into my leg and broke my femur. Now,? Laid up for weeks/months. Going to miss a big family wedding in July. I tend to cry and feel bad because I have already been through so much stuff. It’s ok to feel sorry for yourself. Hang in there and good luck?❤️‍🩹

@jfn which area did u do do shock wave ?

REPLY

@tymlos666
My feet. Legs, thighs, glutes and shoulder have been subjected to Shockwave treatment, and are better because of the treatment.
Diet, exercise, and cranial sacral treatment also, so much PT!
