GLP-1 Drugs Effects on PMR

Posted by christopherc @christopherc, Mar 28 8:08am

I recently read that some GLP-1 drugs like Zepbound can reduce systemic inflammation in autoimmune diseases including arthritis. Has anyone started taking a GLP-1 for diabetes or obesity after being diagnosed with PMR and found that it reduced the inflammation and pain and/or stiffness the disease causes? I know it can be hard to distinguish relief from other treatments like prednisone but I’d be interested in whether anyone thinks a GLP-1 drug significantly reduced their PMR symptoms.

It is a shot I’d do at home, mostly an auto pen , but the last refine was a pre filled needle. Two weeks is the frequency . I have gone to 2 1/2 weeks trying to get to a longer interval but my body strongly tells me no.

@howardgm How often do you take Kevzara and is it an infusion or is it a self-administered home injectable?

I, and most people, take it every 2 weeks. It is self injected. It's a super easy injection process. I'vebeen on it since about 6 mo ths after it was FDA approved. It's great! @jabrown0407

@randersonccc I tried three different statins but all caused a lot of pain so switched to Repatha. It worked like a dream but I had to stop because United Healthcare stopped covering it.

@kathleendw If you really want to use Repartha you might consider getting the drug from Canada - I do that for a couple of my drugs. Google 'Drugs from Canada" and read the Canadian government website about the proper way to do it. The approved drug sores have different prices for the same drug - so shop the drug stores before deciding. Then follow their process.

My Dr is going to prescribe zepbound, I've been on Prednisone since February, so we're hoping it helps with the taper?

I went on zepbound a couple months after starting Prednisone for my PMR because of the weight gain. What I noticed was that the zepbound increased my fatigue. Not sure if it helped with the aches and stiffness and inflammation because the prednisone was already working. I did end up losing 50 lb which I think in the long run really helped with not carrying around extra weight and the discomfort that comes from that. I can tell you,now that I'm off the prednisone my aches have come back and I am glad I'm not carrying around an extra 50 lb

I'm curious about the Novo pill instead of the injections. I've only gained between 5 and 10 pounds but I was already 25-35 pounds overweight before I started the prednisone, with my activity limited by osteoarthritis and prior injuries as well as now PMR and MGUS/SMM (still waiting for bone marrow biopsy results). My primary care doctor wants to wait for the full diagnosis and treatment plan before discussing Novo, but I'm trying to stay positive and proactive.

I started a GLP. Zepbound about a year after I was disgusted PMR. I LOST THE WEIGHT I WANTED TO / needed to and now am on a “ maintenance dose” of 2.5 per week. I do believe that the GLP has helped w my CPAP and my inflammation PMR but it has done much more for me than I anticipated. I am on Actemra and I still do 1-2 mg of prednisone a day. I’m 78 and unless something significant changes I plan on staying on current regimen ☺️

@joyl263 The prednisone has been difficult for me even though it controlled the PMR symptoms. Side effects for me have included anxiety, jitteriness, brain fog, emotional fragility, fatigue, sleep disruption, and feeling generally “off.” Because of that, I am cautious about adding or continuing anything that could potentially worsen GI symptoms, dizziness, nausea, fatigue, or overall functioning while I’m tapering.

It is helpful hearing experiences from people who have tolerated GLP medications well long term while also managing PMR. Do you have any advice for someone trying to balance PMR treatment, prednisone tapering, and concerns about starting a GLP medication?

With your reaction to the prednisone it seems smart to wait until you're off it to start a glp 1. I have to say I initially did not feel good (extreme fatigue)while I was on both prednisone and zepbound at the same time. Our bodies react differently so I hope that you have found a rheumatologist or a primary doctor that you can trust and can guide you
