Anyone have Mixed Phenotype Acute Leukemia (MPAL)?

Posted by jak0627 @jak0627, Mar 31 9:52pm

Wondering if anyone else here has this diagnosis? Newly diagnosed and feeling a bit alone. I'm adult over 50, previously very healthy with no previous cancer. I feel like I've been broadsided physically and mentally. Currently in patient finishing cycle 1 of induction chemo at Rochester. If you have been through MPAL or are currently, would love to chat...

I am sure someone will post. I am going to look up what you have as it is new to me. I had MDS and a Bone Marrow Transplant in April of 2024.I was told i could watch and wait, but i had just had breast cancer less then 2 years before.
My watch and wait could have led to AML.
Try putting MPAL in the subject box above if you have not. There may be posts on it.

Welcome, @jak0627,. I'm tagging @lizzyket to join this discussion. Her husband also has mixed phenotype acute leukemia (MPAL) and can share their experiences.

Jak, as you prepare for stem cell therapy, you might also wish to join the discussions in the Bone Marrow Transplant (BMT) & CAR-T Cell Therapy Support Group https://connect.mayoclinic.org/group/car-t-cell-therapy/

Have you completed in-patient induction chemotherapy? How are you doing?

I was diagnosed with Mpal in November 2022 had three rounds of chemotherapy and a stem cell transplant in May 2023. I unfortunately relapsed in July 2025 and had two rounds of chemotherapy again and had another stem cell transplant this time it was my blood brother sibling October 2025. I’m back to exercising every day and feel great. Each case is different stay mentally strong and as much as exercise as possible.

Hi @jblack1979. Welcome to Connect. I’m going to tag member @jak0627 so they can pop into the conversation with you! Thank you so much for sharing your experience with MPAL and the stem cell transplant! Sorry you had to go through that twice but hopefully the 2nd time is the charm.
@jak0627 is just starting the journey so your input will be helpful I’m sure. You mentioned your 2nd SCT was your sibling. Did you have an unrelated donor the first time around?

@loribmt yes I have a 10/10 non relative match first time round. As my brother was anaemic back in 2022 he was a match but didn’t fit the criteria back then. My mutations were notch1 and PHF6

@colleenyoung I have done 1 cycle of in-patient induction chemotherapy and am in the middle of 2nd cycle (chemo done, but in netropenia). I had a great response to 1st cycle. My next bone marrow biopsy is May 12th with 3rd chemo cycle starting May 14th.

@jblack1979 Thanks for sharing your experience. I'm very nervous about transplant, but know it is best option for me. My brother is over 50 so likely will have unrelated donor.

@jblack1979 I know Philadelphia positive. I think that is BCR::ABL1 fusion?
