Tymlos done, feel susceptible to side effects if start bisphosphonates
Hi! I'm kinda in a bind. I finished 18 months of Tymlos yesterday (good Dexa at one year - -2.3 spine, -2.6 femoral neck, -1.8 hip) and my CTX at 18 months is 165 and P1NP is 32. I have a family history of Barretts esophagus (found out a few months ago when my dad got esophageal cancer - he never had acid reflux symptoms and I don't think I have acid reflux but I do often have some feeling of something in my throat like post-nasal drip). Anyway, I was going to start Fosamax today but, while I don't have acid reflux (as far as I know), Barretts esophagus is inheritable and I don't know if I have that (an endoscopy is the way to find out but I have no symptoms). I am worried about starting Fosamax. On the other hand, I have some slight kidney issues (hypercalciuria and low potassium, and some kidney stones due to Tymlos, all of which I am taking medication for) so I don't know if I should risk doing Reclast.
I don't want to lose my Tymlos gains by waiting too long to start a bisphosphonate. I have a call in to my endocrinologist (I had told her I don't have acid reflux which is true, but I didn't mention my dad's cancer becaue I didn't until now connect my dad's esophageal cancer and Barrett's esophagus which he just discovered due to the cancer to my potential inheritance of it.
Anyway, I am just wondering if anyone has any experience or knowledge of these issues. Would it be wise or not to start Fosamax and see if I have esophageal problems? I'm concerned about how long it would take to get into doctors to even see if I have a problem. If anything, does anyone know how long can I wait before losing Tymlos gains? Thanks for any insight.

You might call your doc and let him know about your dad. If it were me I might want to call my local gastroenterologist office and see what the wait time is in case your doc wants to refer you for an upper endoscopy. It might set your mind at ease in case it goes in that direction. In addition, you can talk with your doc about how soon you would need to start a followup drug. You might do your own research in part by reading what other tymlos users have to say here on mayo connect.
Bone loss can start within six weeks after an anabolic is stopped. I couldn't find a study on how soon bone is lost after Tymlos (abaloparatide) but there are two on teraparatide that show bone loss within a year of stopping treatment without following up with a bisphosphonate.
If you are concerned about reflux, you could do a yearly infusion of Reclast instead.
I was on Fosamax for over five years and never experienced reflux. I was careful to take it according to the directions: on an empty stomach with a full glass of water and waiting 30 minutes before eating or drinking anything.
I had acid reflux when I was younger but that was due to poor eating habits (eating late, eating greasy junk food, drinking alcohol), etc. Once I stopped doing those things, my acid reflux went away.

I have already contacted my endocrinologist (waiting for answer), have already set up a consultation appt with a gastroenterologist physician's assistant Friday. I have been researching like mad online. I was wondering if anyone had experienced something like this, as I can find no clear answers anywhere at this point.

As everything in the medical world takes time, I am concerned about missing the window to start the bisphosphonates, but it seems there is a decent amount of time before bone loss to hopefully determine if I am at risk. I might just start and see what happens, but I don't want to cause damage or a new health problem.
I was told about the specific directions for Fosamax and I am a rule follower when it comes to meds; I was wondering, based on your experience with the drug, do you know if we can drink more than a full glass of water and is exercise at the gym ok to do during the 30-minute wait? I have this also on my list of questions for my endocrinologist when she gets back to me.

1 Reaction@hopeandhope Yes, you can drink more than a full glass of water. And yes, you can exercise as long as you stay in an upright position (no lying down on a mat to do yoga or core exercises). I'd avoid strenuous exercise during that time, though.
