Brinsupri follow-up

Posted by scoop @scoop, Sep 29, 2025

It seems a bunch of us have started Brinsupri. Let's use this thread for discussion. If you are taking Brinsupri have you noticed anything different, including changes to bronchiectasis or side effects? How long have you been on it?

I started brensocatib 25 mg last Sept. About 2 months ago I noticed I was coughing up more sputum while doing airway clearance with 7% saline (which I do morning and evening). At my most recent visit to NJH last month, my CT scan showed improvement for the very first time in over 3 years. Even I could see the difference on the scan! I'm still positive for MAC but my pulmonary function test remains good (ie, "normal", per my doc). I don't know if my bacterial load is less or the same but Doc said that what matters at this point is how I'm feeling overall. So I'm still in the watch-and-wait phase. No antibiotics. No exacerbations.

Bottom line, I think the brensocatib is working! My skin has some new rough patches but that
is about it for the side effects. Yay!

@coffcoff So glad you posted your results on the Brensocatib/Brinsupri.
I am waiting a bit before going on the Brinsupri while I stay on the watch-and-wait begun in Oct. 2023 at NJH. The intercellular I have appears to be down in load since then, Oct. 2023, with sputum tests indicating no colonies showing and no plus (+) number given recently. My last C Scan this past month showed improvement in all areas.
As with yourself no exacerbation, ever, and no anti-biotics started.

Love your "last C Scan also showed improvement."

For me it is just the need to clear nearly constantly the mucus. I realize that is better than not being able to clear but so time consuming and disappointing for various reasons.
I will look forward to hearing again, down the line, about how you are doing.

Thats right, Brinsupri only treats Cyclical Inflammation of the Airways (Dipeptidyc Peptidase (DPPI).
I agree that Antiobiotics will only be required if there is an actual bacterial infection and to prove that is by specific tests, sputum, blood etc.
Alot people will say you have an infection and will require treatment with antibotics, but it does not treat or heal the condition, unless you have an infection.
I dont agree that you need antibiotics for a true exacerbations that is plainly wrong.
