silent aspiration

My cough and mucus production is worse. I don't seem to be clearing after nebulizer 3%saline/duoneb, plus chest compresion therapy twice daily. I had an EGD with a Bravo device implanted to see if I have reflux. I have had previous swallow studies and they have always been normal. Waiting for EGD results. Have been on Brinsupri 25 for 3 1/2 months. I have appointment with pulmonologist in 2 weeks. Anyone have silent aspiration and have any suggestions.

I got my EGD with Bravo results which show no reflux disease. I had radiation therapy in 2021 for neck cancer. The GI doctor now believes that the coughing is due to the radiation therapy. I see the pulmonologist in 10 days. Any suggestions as to what questions I might ask. I have alway suspected that the radiation might have been a causative agent but if so, what is the treatment? Seems like a swallow study would have shown if I were aspirating? But the multiple studies have been insignificant. Any input would be appreciated.

I would be interested to hear the results of your reflux test. I am sure I have silent reflux, diagnosed with gastritis, was once told I could be in line for Barrets Oesophagus, was put on PPIs for years but then I went off them. About 18 months ago I had a barium swallow which said no evidence of reflux.

Also I am a huge producer of mucus with lots of coughing.

@narelled23 mucus, coughing, and hoarseness can be associated with silent reflux, among other things.
I experience this with my silent reflux/LPR.

Thank you. Planning a trial of a H2 agonist shortly to see if that helps mucus volume. However have just been diagnosed with mod/severe COPD and a new inhaler which seems to be reducing mucus production also. Will have to wait and see how it settles.

H2 Agonist is a drug which reduced stomach acid...you can google it.

The new inhaler is a LABA/LAMA inhaler for COPD called Anoro. I was not told it would reduce mucus but some side effects are drying ones so I am wondering ifcthat might result.

Sorry...you are right.

Probably going off the Anoro as it is affecting my sleep...for 10 days now.

@narelled23 You are correct. That medication is causing the mucous to become dry in your lungs thereby not being able to bring it up. Hopefully the 7% saline nebs and ACTs will moisten it enough to bring it up. It’s a difficult balance when the medicine is necessary to help you breathe properly.

Have you had experience with Anoro? Because I haven't been able to sleep properly for last 10 days I am stopping it today. Trying to get direction from my specialist...however a daily antihistamine seemed to be helping my shortness of breath before I started the Anoro, I am hoping that will still be the case.

No, I have not . I have Bronchiectasis and COPD. Due to the drying effect of histamines and some inhaler medication, I just do the albuterol followed by 7% saline men’s and ACTs to remove as much mucus as possible. I’ve also been on Brinsupri for 2.5 months which has overall decreased my sputum production and inflammation.
