Cataract surgery and lens choices

Posted by nathanstrahl @nathanstrahl, Mar 9 10:20am

I am scheduled for cataract surgery in about a month. I have been given numerous options for lenses, but remain confused as to the best choice. I have very mild astigmatism in one eye and very little astigmatism in the other. I have moderate dry AMD, but no real vision deficits noted. Money is always a factor but when it comes to my eyes it is much less a factor in choice. My goal is to be able to have far and intermediate vision without glasses but I appreciate I will likely need glasses for reading. The lens that comes standard with Medicare is one option and Torex is another option. Again, putting price aside, I want to limit my dependence on glasses and am willing to pay extra for that.

Thanking yoi in avance.

I have been prescribed eye drops such as LATANAPROST; NOW TIMOLOL MALEATE for eye pressure
I seem to have extreme fatigue, depression ,tiredness; and chills at times also
should I go off these, or are there other solutions to this condition??

Welcome @tonyfanm We are just patients like yourself and cannot diagnose or give medical advice. I don't think I would stop taking any of my medications without first consulting my doctor.

Have you talked with your doctor about your symptoms?

....have eye issues but at 82 am hoping dont have to have this surgery... its just am dealing with other illnesses .. but I am curious and its inforamtive to read these comments... may I say that about 10 yrs ago (Ontario) my neighbour after our one and only ophthalmologist asked her if afer this surgery she wanted to see far or close without glasses.. she said she would prefer to not have glasses as a senior but did a lot of sewing and cooking etc., and as she didnt drive etc. would get glasses for distance. For some unknown reason the ophthalmologist did the opposite! We dont hae a lot of specialists where we live and once it was done A. didnt see any purpose in complaining about it... so would it be a good idea to double check with the eye surgeon on "the day" to ensure he does what the patient wants,,, and, does this happen very often. Dont want to scare anyone but as soon as i read or head the word cataract my mind goes back to what happenbed to her. J.

@tonyfan Hi jsut to say I have been using a form of Latanaprost since diagnosis of "normal pressure glaucoma" with narrow angles, since 2018... mine has no preservatives and is called Monoprost. I also use dry eye drops. There definitely are side effects with most eye drops and quite a bit of info on Internet but I agree that it is not wise to go off any of them without consulting a doctor or Ophthalmologist.. whoever gave you thepresdcription.. I was told not to stop mine for even one evening. I did read about a patient who had depression while on Latanoprost and it went away when he changed but nothing about this in Canada or USA news.... meds in eyes can affect us so hopefully you can find the one drop with least side effects .

@tonyfan ....curious after reading your post as I worry too... and agree no one can diagnoze here and I am just agreeing never to stop glaucoma eyedrops without checking with the prescriber.... but I recall when Dr.Trope from Toronto Western Hospital, head of Glaucoma Dept, told my local optometrsit not to give me a certain drop due to my history of depression but I cant for the life of me remember which one (2018 and he has retired and i dont have same optom) I just checked Internet and you can read for yourself that Timolal and Betaxolol "can" cause depression fatigue and even confusion in older adults. Please contact yuour eye dr as there are other eyedrops, and there could be an alternative which suits your body better... unfortunately we cannot let the eye pressure climb... bestof luck

As a side note, I selected lenses meant to enhance night driving performance. Each lens cost me about $800 in 2020, which is more than what the health service covers for regular lenses in Canada. During my visit to my usual optometrist, he discovered that the lens in my left eye was designed to help with close-up vision. The specialist expressed irritation that I had that lens, pointing out that it was much pricier than what I had originally purchased, almost as if he was blaming me for it. I said, "I suppose they must have made a mistake," to which he said, "We don't make mistakes". It's amusing because it's clear they do, indeed.

Before my cataract surgery last fall, I wore contact lenses for many many years. Most recently I had dual vision contacts, meaning one contact lens was for reading and the other lens was for distance. Because of this, I was able to do the same thing with my cataract lenses -- the lens in my right eye is weaker strength so I can see up close pretty clearly. The lens in my left eye is for distance. It works so great for me that I don't even need cheaters for reading, which I needed prior to my cataract surgery. I am getting glasses for driving to bring my right eye up to 20/20 for distance. Anyway, I know that's probably not an option for you, but I wanted to share my experience as I am so pleased with the results.

@daisy17 Hi! I had the same type of lens implant as you approximately 4 years ago. I wore gas permeable contact lenses for 50 years with the last 10 or so years as monivision. It was a no brainer for me to choose my implants in the same style. I also have glasses for driving at night. It’s so wonderful to be able to see the tiniest print on a label! Perhaps not everyone’s choice of implants but like you, I did not want readers. 🎶🇨🇦🎶

...the doctor who prescribed Latanoprost might prescribe another drop .. they all seem to have some side effects; also I am in Canada but USA has a Glaucoma Society maybe email them? Or possibly speak to the Pharmacist at the pharmacy where you got the drops? My pharmacy had a question about drops and actually Faxes the Specialist ,,,, best of luck with everything! J.
