MRI Results

My MRI results seem to be more optimistic than the original diagnosis of Intermediate "unfavorable risk" given from the biopsy. I have gone through it and AI searched the various terms. Have a consultation with an Emory Proton Beam Radiation Dr. tomorrow and with my Urologist/surgeon on the 20th.

MRI OF PROSTATE WITH AND WITHOUT CONTRAST
CLINICAL STATEMENT: Elevated PSA level
TECHNIQUE: Multiparametric MRI: Axial: Small FOV T2,T1, diffusion, full
field of view T1 FS. Corona and sagittal : Small FOV T2. Multiphasic
postcontrast axial T1 FS vibe following the uneventful IV administration
of 7.3 ml of Vueway.
COMPARISON: None.
FINDINGS:
PROSTATE MEASUREMENTS: 4.3 x 2.6 x 3.4 cm; volume 20 cc.
PERIPHERAL ZONE:
- Probably prostatitis or sequela thereof in the peripheral zone
bilaterally with linear/wedge-shaped T2 hypointense signal (PI-RADS 2).
More focal T2 hypointense signal the right posterior peripheral zone of
the mid gland measures 1.2 x 0.8 cm (5-15), appearing slightly nodular
on sagittal images (6/20, 21). There is associated moderate ADC
hypointensity
TRANSITION ZONE: BPH changes. PI-RADS 2
EXTRAPROSTATIC EXTENSION: None.
SEMINAL VESICLE INVASION: None.
PELVIC NODES: No suspicious adenopathy (short axis 8mm or greater).
BONES: No suspicious osseous lesion.

IMPRESSION:
1. No PI-RADS 4 5 lesion identified
2. Probably prostatitis or sequela thereof in the peripheral zone
bilaterally. There may be PI-RADS 3 signal abnormality in the right
posterior peripheral zone in the mid gland. If there is persistently
elevated or rising PSA level then prostate sampling may be beneficial

Similar situation as you with negative MRI. What was your PSA? I am surprised that you were able to get a biopsy on free pSA test results. Its good you did obviously. My PSA was around 2 and with negative MRi , I had to offer to pay to get a biopsy here in San Diego.

@beachflyer My PSA when diagnosed had gone from under 4 to 7.1. I think the Free PSA test is what my Urologist used to get approval for the biopsy. I thought when I got the negative MRI that I was in the clear....but no such luck!

Feeling almost 100% back to normal. I pray things remain this way and my PSA stays low.

@beachflyer My PSA when diagnosed had gone from under 4 to 7.1. I think the Free PSA test is what my Urologist used to get approval for the biopsy. I thought when I got the negative MRI that I was in the clear....but no such luck!

Feeling almost 100% back to normal. I pray things remain this way and my PSA stays low.

@capatov Do you remember if they did IMRT-SIB wherein the hit the seminal vessels at the same time? That is what I was offered due to my negative MRI, low PSA, Gleason 7 (favorable). They were concerned that without good scans the PC could have traveled. I was also offered SBRT, but they also wanted to hit the prostate bed. The low PSA expression also added some issue in tracking for BCR while PSA fell to NADIR so in the end, the consensus was "getting to PSA zero" through RP. Good news is no incontinence, but partial ED still at 16 months. Excellent news on your part about the lack of side effects and PSA numbers !

@beachflyer I do think I may have had some seminal vessel involvement. I don't know if my IMRT was SIB? They did the follow up high dose brachytherapy "boost" on top of the five weeks of IMRT. I also had six months of ADT while the treatment was going on to increase the chances of an effective treatment program.

I never wanted surgery so I was very pleased that the Urologist and RO felt my radiation based program would be effective.

Now 10 months post treatement...my PSA is at .01. My treatment team seems pleased as am I.

@ezupcic Shame on me but I did not get my actual Free PSA "total PSA" score. All I remember from my Urologist was "your FREE PSA results suggest up to a 50% chance of PC" which is why he was adamant that we do a biopsy.

My PSA at time of diagnosis was 7.1. Now 10 months post IMRT + HDR...my PSA is a .1
