Anyone else on Nucala for EGPA having sinusitis issues?

I have been on Nucala for EGPA for 15 months with good results while still on prednisone. I tapered slowly over 9 months and successfully stopped taking prednisone by August. By October my sinuses started acting up, exhibiting as inflammation of my sinuses, loss of sense of smell, difficulty sleeping due to blocked nose, inability to clear either by blowing my nose or post nasally. I have tried saline rinsing, Allegra, Xclear nasal spray, all to minor improvement. Halls sugar free cough drops will sometimes lessen the inflammation enough for broken sleep. I tried budesonide nasal spray which helps along with cough drop which helps me get some sleep.
I tried a short taper of prednisone back in January which solved all issues, only to have them return when I stopped. Wondering if this is a side effect of Nucala or a break through of EGPA? The notes on Nucala mention “ noisy breathing” and way down in lists “ unknown issues with congestion”.
Better than having the full blown disease, but wondering if anyone else has noticed this?

I just started Nucala for EOS Asthma. I've only had 2 shots but developed severe nasal congestion shortly before the second shot. It's now been almost 4 weeks with no improvement and over the counter medications have not helped. I am seeing my Doctor next week but am also wondering if this is related to the Nucala.

I am still doing very well after 2years but my sinuses are still an issue. I have refused a suggestion of surgery for a 3rd time since the first two surgeries did not solve the problem . I have now discovered I have active Silent Reflux which is different from Gerd and responds much better to Pepsid for a start, morning and night, but most especially following a low acid diet. Read the book Dropping Acid by Kaufman and learn how to heal your esophagus, sinuses, voice. It is a long process and I am hopeful there are answers that are not strictly medicine related but life style. I do miss chocolate though!😵‍💫 Silent acid can even affect lungs. Nucala is a great treatment for eosinophilic asthma and EGPA ( which I developed) but sinusitis seems to be a separate issue that needs other exploration. Best of luck in unraveling your problems and know there are others trying to dig their way out along side you. Good luck at your doctor’s appointment.

I have had the same thing happen to me with dupixent. I needed to cough up mucus and couldn’t my nose was so congested i couldn’t hardly breath in my oxygen. The only running i had was post nasal drip and lot of it. My sinuses were so dry, like a desert.
I have since been off due to fix him for a couple of weeks due to bronchitis. I couldn’t hardly even cough up anything I couldn’t cough up mucus. Everything was like dried up in my sinuses all the way down in my throat couldn’t blow my nose. nothing seemed to help it, not even the saline. I was concerned I had silent reflux so I scheduled an appointment to go to Ent Doc he run a little tube and light up my nose down to look at my throat and he said he’s saw no sign of it although I have the symptoms of it go figure. Go back to my doctor on the 17th. Not sure I want to go back on the dupixent either cause I don’t like all this dryness. I love because I could breathe good and it helps the inflammation but again I don’t like to dryness up in my sinuses. With COPD when I need to call pup stuff I wanna be able to cough it out.

I think you are right about the dryness being a problem with sinusitis, and many of the meds used to treat sinusitis do cause dryness.
I had a rough winter and wound up on prednisone twice to get through the winter and a bout of heavy thick, yellowish sinus discharge and coughing up thick clear mucus. Urgent care prescribed a 12 day taper starting at 30 mg which , of course, cured all aches and cleared sinuses. I stopped all doses of Astepro, Nasal chrom, sinus saline sprays, etc. When the prednisone was finished I realized my voice was gone and I had vague signs of Thrush in my throat and probably on my vocal chords. This happened once before. I am using Nystatin oral swish and gargle and swallow, snd my voice is much better. I think we need to concentrate on not getting our nasal tissues too dried out since it makes them prone to infection or fungus. It will be interesting to see what happens when I finish this weeks regimen on Nystatin. I think I will try sinus salt rinses as needed and try not to use too many products to stay breathing! Simple things like XClear nasal Spray andNeilMed NasoGel for dry noses are gentler products that may work better than the big guns!
