Post aquablation & an overactive bladder

Posted by Flipboy59 @bobpuli, Jul 28, 2025

I just had aquablation 4 weeks ago and now I have an overactive bladder. Does anyone had this procedure done and had the same outcome?

@joseflesh You're only 5 weeks post aquablation, way too early to be panicking. You're experiencing many of the same issues that many of us faced and you'll continue to have ups and downs until you get to 3 or 4 months with continued lesser improvement to follow. The pain during orgasm is not uncommon and should get better, mine felt like a spasm in my lower groin. I had the same problem which took about month 3 to go away just like my doctor told me it would. I had a great recovery other than that until about 8 weeks when my urgency and frequency suddenly increased. I thought I had a urinary tract infection at 10 weeks, but my urine tests showed leukocytes and no nitrites exactly like you. The leukocytes can be just due to the inflammation in your prostate not necessarily infection, but a simple urinalysis will answer that question. It also showed red and white blood cells even though I could see no visual indications of blood. I don't think any of that is uncommon or a reason for major concern, but check in with your doctor. I'm at 3.5 months now, the pain during orgasm is gone. I still have some minor burning in my perineum area and a little irritation in my urethra some times, but I think that will continue to get better over time. You just had major surgery on your prostate, at 5 weeks don't expect too much, but by all means check in with your doctor. If it helps your experiences are very similar to my own. We all are own journey and I think how old you are, how long you delayed treatment (and damaged your bladder) and of course a little luck all play a factor. You'll read here cases where guys have had poor recoveries and others where they were complication free and never looked back after 6 or 8 weeks. Keep the faith, I think you're going to do great, but it's going to take a little more time.

Thanks so much for the words. It means a lot. I am 57 years old. I decided to go through this procedure now rather than later. My prostate size was 65, roughly double of normal.

It is comforting to know these symptoms are not outside of the ballpark for this procedure. Urgent care prescribed CIPRO. The urgency had decreased somewhat. The burning still there. I am taking the last of the anti-spams medication the doctor gave post op.

I am looking forward to a positive outcome and provide support to others through this forum.

@joseflesh yeah I also went to the urgent care when the urologist said I didn't have an infection and they also gave me cipro. Great minds! You're on track, good luck. Smart move getting ahead of it, I wish I had it done earlier, I'm 64 and was at 94cc with a large median lobe.

I am more relaxed knowing there other "Aquablation brothers" that has gone throught similar experiences. My urologist told me to resume sexual activity once I feel better. However, I am going to wait at least 2 more weeks and test the pipes again. I want to avoid pain as much as possible.

As of now I am not urinating every 10 minutes. When it happens, the urgency is extreme to the point that if a bathroom is not near by I could have not hold it. The burning is bad, and the pressure towards the end of the stream is not fun to go through. Those are my symptoms as today. Oh, I forgot! My bowel movements have not been the same since the procedure. However, this has not been bothersome. Week 6 will be next week. Hopefully, I will start to see some improvements to the status quo. I will keep this forum updated. Stay strong!

UPDATE!

I am 7 weeks post op (Nov. 4th). The symptoms that still lingering are the burning when urinating (accompanied w feelings of doing #2), urgency and sometimes the frequency. I believe stress can affect these as anxiety kicks in because of the eagerly feelings of getting better. There are good days and not so good days.

Everything else is working as it should. My urine stream is strong depending of how much liquid is in my body (due to frequency). My ejaculations are way better than before the procedure.

I have an appointment w urologist in January. Hopefully, there is nothing wrong as these lingering symptoms are bothersome and worrysome thus affecting my lifestyle. I have to plan for when urinating before work meetings or doing errands. I have not attempt to start exercising for fear of making my recuperation worst.

Please keep the faith and stay strong. Happy Holidays!

I am seven weeks out of Aquablation. I am having an intermittent overactive bladder situation as well, sometimes I can go two to three hours without peeing at other times it's much sooner. I had a follow up appointment this week and I was prescribed Solifenacin 10 Mg. I haven't taken it yet. I don't want the confounding of healing naturally or due to the medication. UR said it's expected and that I am healing well.

I am experiencing the same issues. I guess some of us take longer to heal. My prostate size was 65cc, not too big, but it seems the bladder still adjusting. I have a follow up app in Jan. Hopefully, there is nothing wrong or out of the ordinary.

@joseflesh You might try Azo Urinary Pain Relief. It's over the counter but is the same med they gave me in the hospital. If you can, keep your doctor in the loop. Of course Tylenol might help to.

Take Care, Randy

Thanks for the advice and encouragement. I am most concerned if burning and pain still "normal" after 7 weeks post procedure. My appointment is not until the end of January. I will try to see my Urologist before that time. I can understand the frequency and urgency, but the burning and pain is getting old. This is not fun. Everything I have read points to a lingering swelling of the prostate as the culprit. It truly takes a toll because it does not go away, nor it seems to improve.

I will call next week to see the urologist ASAP. More to follow...thanks!

@joseflesh I think recovery varies for less of people. I was doing great at 2 months, but frequency and discomfort increased in month 3. Now almost 4 months the frequency has settled down mostly but irritation in urethra and burning in perineum still an issue so you're not alone. My next Uro appointment wasn't until 6 months, but I scheduled something for next week to discuss any options. I still find a lot of heavy activity increases symptoms. Still all in all in much better of than I was so i still feel confident. My flow is generally very good, up less at night and frequency isn't bad

I think the intensity of activities has a direct correlation with symptoms. Taking care of regular home chores( vacuum, etc.) do increases discomfort. I can tell my bladder, pelvic or prostate is (are) inflamated. Again! I just want reassurance from my Urologist that these symptoms are part of what's considered normal.

I do live alone and have to take care of my 2 cats and home maintenance. My girlfriend is out of State and unable to assist due to work. Nobody wants to be sick, nor in pain. That's my only complain, the pain urination, burning and irritation overall.

It is comforting to know there are others with similar symptoms. It gives me hope that my struggle is within what's considered normal recuperation.

I am positive we will reap the full benefits of Aquablation in not too distant future.

Hi Everyone,
I’m scheduled to have Aquablation for BPH in 4 weeks. I am very concerned about bleeding after the procedure. I asked my surgeon what he will do to control bleeding. He said that he does spot cauterization only at the opening of the bladder neck area. He said he doesn’t go into the prostate area for cauterization because of the risk of impotency. When I go to AI, it tells me that this surgeon is not skilled enough and cauterization should also be done inside the prostate. Do you know if your surgeon limited cauterization to the bladder neck area or should it be done the way AI suggesting? I appreciate any feedback. Any of you have. Thanks so much!
