That's a very excellent, and probably a telling, question. You should get this conundrum solved by contacting your service for the pacemaker or your family physician or cardiologist. This is a mismatch. Of course, your wearable device might be wrong..... This might be a time to invest in a wearable cuff, either plug-in or rechargeable, that you can put on your arm and see what the pace looks like. If it's rapid, it is fibrillation or flutter, or tachycardia.

1 Reaction@peter800 I have a very similar problem. My pacemaker is set to pace me at 55 and my apple watch gives me low heart rate notices of less than 40. My cardio pinged my pacemaker and said I was experiencing many PVCs per hour and the watch probably wasn’t counting the extra lower heart beats only the upper beats. Upper heart beats once and lower beats twice per upper beat so the watch only counts the upper beats. Not sure he’s an expert on Apple Watches but it made me calm down.
Thanks for your response. That appears to be the right assessment. PVCs ( apparently safe if not too many) can disrupt the signal. But taking my wrist pulse was also low when Watch was saying low. That I don’t get.
The positive is that I have no negative symptoms when this happens.
