Issues after Nissen fundoplication revision

Posted by refluxrefusenik @refluxrefusenik, May 23, 2023

Hello. I had a Nissen redo two years ago, and about a month ago I started to have some epigastric pain. It started with some epigastric pain sort of behind the base of my sternum, sort of like light stinging pain. I had some unusual throat tightness for a couple weeks followed by some unusually frequent burping. With both this redo and my previous Nissen, I could never really burp much, but that was always expected.
However, over the past month since the incident, I've been having mostly small burps (a few larger ones) close to a hundred times a day without exaggeration. Even if I drink water, I feel an awkward discomfort in my esophagus and have to burp, often multiple times. I don't feel like I'm having discernible reflux, but I do have a consistent discomfort in my esophagus. PPIs haven't seemed to help at all.

Has anyone else had any issues of temporary irritation where this has happened? I'm hoping it's a transient issue that could be linked to something else, but I'm not sure what else it could be. I'm in the process of moving and the company I worked at went under recently, so I'll be unable to see a doctor for at least a few months. In the meantime, I figured it would be worthwhile to see if anyone else who has had a wrap has faced an issue like this. I'd be especially curious to hear if anyone else had an issue like this and— if it went away after a time— what caused it.
The wrap was done by an experienced surgeon who said the durability should last for a long time, and I've been careful to not do anything that would cause too much abdominal pressure.

Anyone else have a similar issue?

Hi, first time on the site. I am considering having the Nissen fundoplication procedure done. My GI doctor has indicated after an endoscopy bravo test that I have significant inflammation where my esophagus meets my stomach. I have been taking a PPI for years and have absolutely no outward feelings of heartburn, regurgitation, etc., The reason I first went to him was due to a persistent cough and throat-clearing. My ENT suggested going to the GI doctor - thus the test and his diagnosis. I have never even heard the term "silent reflux" before a month or so ago. So now I am weighing the pros and cons of having this surgery. I'm wondering if I simply improve my diet and lose weight if I had hold off on the surgery?
After reading some of the complications and side-effects of the surgery I am not sure which way to go. Can use some advice?

Hi, @nomadnurses2 - welcome to Mayo Clinic Connect. As you were talking about significant hair loss three years post-Nissen Fundoplication, I've moved your post here to a discussion where others are talking about issues following this surgery:

- Nissen Fundoplication 2 Years Later: Complications? https://connect.mayoclinic.org/discussion/nissen-fundoplication-2-years-later-complications/

Hoping others here who have talked about being on the other side of this surgery will provide some support and share whether hair loss has been part of their experiences, such as @metwo @dolphinpoo @pat23 @jord @jayjay1969. I'd also like you to meet @jlharsh.

I see you said that no cause has been found for the hair loss by a dermatologists and an endocrinologist. Wondering if they suggested you treat the hair loss? If so, what treatment did they recommend?

Thanks for the invite @lisalucier. You have amazing timing!

Hi @nomadnurses2 . I am familiar but haven’t had a Nissan Fundoplication procedure. My mom actually had hers done years ago so I am aware of challenges following. I don’t remember hair loss but now I am going to ask her.

My digestive symptoms haven’t led to a diagnosis at this point but I do have them. And, I keep losing hair. It has been little by little but literally, I think I will blame lisalucier for bringing it up. Two days now I’ve had a bunch of hair ending up covering the drain after my shower.

Is your hair loss gradual all over thinning or spots on your head? I am interested to know what you learned from a dermatologist and endocrinologist as well. Should I pursue?

Has anybody else had problems resulting from fundoplication surgery? Namely, twisted esophagus and gastroparesis? How do you deal w/ it?

Mine was wrapped too tight and now I can't swallow food without drinking after and building fly until food goes down. Told nothing could be done.

@viper84I had the Nissen surgery two years ago. The first few months were OK. I then reverted back to my old symptoms. I take two pills a day and they only help with the “burn” from reflux. Now I have stomach pain and scheduled another endoscopy. I have dealt with constant throat clearing before and after surgery. I believe I should not have had the surgery.

@lillymo1 I imagine you are frustrated going through feeling better after having Nissen surgery only to go back to your old symptoms and second guessing the surgery decision.

I am linking @viper845 like you did. I notice for some reason a link was not created for them in your comment.

You mention having another endoscopy because of stomach pain. Is this the same pain you were referring to or is this new? When is your endoscopy?

Was going to watch a movie but fell down this rabbit hole while looking for information on whether Emma supplements are a scam or actually helpful (probably a scam). So glad I found this website because I’m learning so much.

I had the Nissen in 2012 and benefited from it greatly for most of the last 14 years. Of course, I didn’t burp for at least 10 years after the procedure and now only burp slightly if I hiccup. As a result, I also have a LOT more gas than I had prior to the Nissen, along with horrible bloating which has been getting worse the last few years.

I’ve also had problems with constipation for about 50 years now, but managed it mostly with laxatives, prebiotics, probiotics, etc., most of which helped minimally. A couple of years ago, I had to have a tooth removed, and the oral surgeon noticed I had thrush. He prescribed two weeks of fluconazole, which worked like a miracle because I started having regular full bowel movements. I thought I had found the cure for my chronic constipation! Unfortunately, that didn’t last due to frequent bilateral ear infections for which my doctor insists I need antibiotics. So, even though I take a round of fluconazole after antibiotics, I have not had the same relief for constipation the last couple of years.

More recently, nothing seems to be working for my constipation so I was happy to read about gastroparesis. I had heard the term before, but knew nothing about it. I don’t have all of the symptoms but enough to suspect that might be the problem. Naturally, I’ve had to eat much smaller meals since 2012 but now I can’t even eat that much without feeling extremely bloated and having what feels like a 6 inch strap around the top of my abdomen. I can’t even wear a bra anymore because it’s so uncomfortable.

I’m really grateful to all the people who have posted about their Nissen experiences and I’m going to check out several things mentioned to see if I can’t find a resolution for myself. I’m not even sure I’m getting enough nutrition daily to support my life. I get so tired but, there’s no way I can eat more or drink more water (for God sakes!) so that I can feel better.

For me, at least until the last couple of years, having the Nissen was a godsend. I had been on PPI’s for 10 years before and my Gerd was just getting worse and worse. In fact, my doctor at the time put me on a double dose, which did not help at all and seemed to make things worse. I hated having to sleep sitting up all the time; not only that, I was diagnosed with osteoporosis about four years ago, which, I’m convinced was at least in part due to being on PPI‘s for so long.

I can’t give anyone advice about whether or not to have a Nissen, but I know it helped me for a long time. I just hope I can now find some relief from what appear to be long-term side effects of the procedure (from what I’m reading). Thanks to all of you for sharing and to Mayo for hosting this forum.

@jlharsh I am scheduled for the scope in mid-April. I sometimes have quick sharp pain in areas of my stomach. I never had this pain until recently and although not bad, it is concerning.

It’s been almost 3 years since you posted so wondering if you ever got a resolution. The only similarity to your description that I’ve had is the behind the sternum pain (intermittently).

I can’t imagine experiencing the amount of burping you described - it sounds painful. Hope you’re doing much better now!

I am tagging @refluxrefusenik who I assume you are addressing in your comment. It is interesting that you have such similar symptoms. I agree, it does sound painful.
