Has anybody successfully tapered off of lorazepam (Ativan)?

Has anybody successfully managed to successfully wean off of Lorazepam?
If so how was it done?
And if it was by way of switching to other drugs, were you then able to taper off of those entirely under a doctor's guidance?
My husband has been on Lorazepam for 16 years
He did well for 14 years, but dragged his feet about trying to withdraw while he was feeling better.
In May he had a relapse of anxiety, and insomnia, with brain fog, heart palpitations, and more.
Currently he's doing better, thinking better, but still feels like crap a large part of the time, can't sleep without the drug, can't nap.and still struggles with anxiety.
At this point, I believe it's the drug that is actually causing the problem. Currently he's doing 1 mg at bedtime and 3/4 mg when he wakes up at 2:30 am.
I know it takes 2 years for the brains GABA receptors to normalize after taking Benzodiazepines.
Also know it's really hard to withdraw, has to be done extremely slowly, and the side effects are exactly the same as the symptoms it is meant to help.
My goal is to eventually wean him off of all drugs as functional health tests show they block his nutritional absorption and may be damaging his intestinal lining. I would like to be able to repair his health using nutrition, supplements, and neurotransmitter amino acids.

It’s a very insidious drug and doesn’t always like to cooperate with our plan to stop. I remember when I first started taking this drug, I could take it on and off without any problems and I even stopped taking them for a while but before I knew it I started feeling physically ill and I wasn’t even sure what was causing it until one day I took an Ativan and everything went away, I should have stayed off them but I just couldn’t cope anymore…that was over twenty years ago. My advice to you is start now with your withdrawal, you’re in a good place to begin a slow taper, you mentioned that your dose is 0.5 mg. so I would recommend cutting it down to 0.25 mg and adjust to this for perhaps 4 weeks or more depending on how you feel, the main thing is to go very slow to avoid relapse. There’s also lots of good info online that can be very useful. I think that you have a good chance to get off them and perhaps if needed ask your doctor to give you something else that’s not addictive to help you cope if needed, best wishes for your recovery.

Thank you so much for your suggestions and input! So i did do some research on the ashtons manual after yesterday’s experience. I was convinced my body had entered fullblown withdrawls because the panic was so intense and it was somehow coupled with this dark sense of impending doom and depression which i dont typically suffer from. Not sure if it was withdrawals or rebound anxiety or what?! That was 25 hours ago. I havent used anything since and my plan is not too. I have had minor bouts of anxiety throughout the day but been able to redirect my thinking thus far. Made it to the gym this morning which i imagine can only help throughout this process— i go 5-6 times a week and will be CRUSHED if physical symptoms from stopping this crap deter me from making it. My thought process behind not tapering is that i wasnt a daily user (albeit the past 9 days) .. and given the low dose i was hoping i could get away with just stopping. Am i crazy for thinking this is possible? Im open to cutting in half and tapering if absolutely necessary but havent felt that necessity today (yet) fingers crossed

@jebz88 First, I'd like to welcome you to Mayo Clinic Connect. I see you have already received support that you've found helpful. I understand the struggle with anxiety and trying to figure out what works best.

While I mostly understand how benzodiazepine medication such as Ativan works I can only comment on my own experience. As @frouke commented, this class of medications is insidious. You are very wise to be concerned with tapering off and how to do that. For now, and from what you've written the anxiety and panic attacks you experience ramp up at times. I went through that same scenario when I first experienced panic attacks. This was over 30 years ago and I worked with a psychiatrist who helped me find an antidepressant (desipramine but there are better antidepressant medications with fewer side effects now available) medication that worked to bring down the anxiety and the depression I was feeling so I could better cope. Since I took the antidepressant daily I didn't have to "react" and worry about panic attacks. I learned coping skills for when I did feel anxious.

Are you already taking antidepressant medication that works for depression and panic attacks? There are many medications that your doctor can try with you to see what works best. It takes patience on your part to go this route as it takes time to get to the dosage level that works best for you and for your body to adjust.

There are other medications too that are not in the above classes that work for anxiety and depression.

Thank you for your input and suggestions! I am not on a daily med and my overall goal is to stop this and not start anything new in the process. I dont want to rely on a med to feel “normal”. Im not totally against it but want to try this approach first - get a therapist and continue using the gym and implement meditations more and see if that can get me back to my baseline. Prior to this medication i would go through phases of panic attacks that could occur on and off for days — sometimes up to a week. BUT these periods would only happen a handful of times a year— 2 or 3 max. I definitely have social anxiety and my bodies fight or flight/ adrenaline seems screwed up at times - but with that being said i feel this med has become a crutch and is being used for some its intended purpose of panic attacks but also being used for general anxiety and stressors. Which was never the plan. I dont like the feeling of relying on meds for internal peace if that makes sense? Sorry for the long winded answer 😂 i will definitely be seeking a therapist for coping mechanisms. Thank you!!!

I can relate to what you’re saying, you can try it out and see if you can cope, I agree that it’s been a very short time that you have taken them and not on a regular basis. If you do feel uncomfortable then you can get something mild to help you through it, only you can really determine what to do because it’s your body, also it takes Ativan two weeks of consistent usage to get dependent on them and you didn’t take them every day, best wishes and good luck.

I was given 30 years ago 6 mg per day. 3 years later I discovered what kind of med. I’m taking and since I’m fighting to lover the dose. Last 4 yers I was on 2 mg. 6 months ago I started slowly go down and I’m presently on 0.5 mg. Next months will be 0.250 mg. And this is my last dose. Pray for me, because took me 30 years to quit and my last step seems to be very challenging. I’m positive even my body and mind goes through Hell. Sides effects of withdrawals are hard to survive but I’m not giving up. Believe me even a low dose like 0.250mg can be hard to reduce. I think it’s more in my head that actual need of it. I’m taking Lion’s Mane drops And Ashwaganda to replace ativan and it helps. Good luck to all of you.

Psychiatrist pxed .5 mg twice a day of Ativan in conjunction with 100 mg of bupropion twice a day for major depression and anxiety. I am 77 years of age and the psychiatrist provided NO information or caution with regard to either drug. Aftertaking the meds for 30 days I went online to check on them and only then did I discover that I may already be hooked. I would like to taper off the Ativan, but I am concerned about the side effects and my age. When I mentioned this problem to the psychiatrist who is also in his 70's he said he has never had any patients who had problems and said he has had many patients on Ativan "long term." I was using the Ativan primarily for sleep. I have continuous rumination in my sleep and wake up with the thoughts. Cannot shake them. Anyone else in a similar situation? What do you do?

It was tapered from 4 mg per day. It was hard yes.

Been prescribed ativan since end of October so almost 11 months. Was prescribed .5 mg and typically would use it around 2 weeks out of a month sometimes more. Sometimes i wouldnt take it for a week and then others i would rely on it for a week straight. This has been one of those weeks. A couple times a year i get what i call “intense daily panic attacks” — not typical anxiety but the kind of attacks that feel debilitating. Ive taken it for 9 days straight. Today i was committed to not taking it and attempting to stop it entirely but the panic came on so strong and i became immensely depressed and on edge and couldnt sit still but also couldnt lay down. I couldnt be a parent to my children! So i caved and took one. I just find it weird because previously i could go a week without or 3-4 days without.. im talking just last month and would have no issues or symptoms of withdrawal. So what gives? Did i magically become chemically rich or physically dependent in the past week due to 8 days of use in a row? If so.. what suggestions do you have to taper off and be done. I assume it wouldnt be a long taper given the fact its a low dose for 10 months and not an everyday thing. Dont want to have to rely on this med for another moment so any suggestions or experience is appreciated !

I can understand that it’s scary and you’re not alone, I personally don’t want to stop taking it because it works very well for me, I have tried so many other medications and they just didn’t work for me. I never really knew that there’s a big problem going on until I came here and read all the stories that people told about their experiences, I also realize that they were being pressured by the doctors to get off them and that’s what was causing this. I don’t know why the powers that be are doing this but I’m sure they have their reasons, I don’t want to fix what isn’t broken and hopefully I won’t get pushed into this very difficult situation.

Sorry if my post is inciting fear. Was not my intention at all just seeking lived experience and input, but my apologies! I think if the med works for you and you need it then there is nothing wrong with continuing use! I am wanting to get off because i feel ive become dependent on something that was initially supposed to be a short term med to help with panic attacks. I dont feel i need it daily anymore if that makes sense. Thanks and best of luck
