What could be causing my constant feeling of head pressure?

I have a constant feeling of head pressure 24/7 that has lasted for months. The pressure is all over my head except the back. It improves a bit when laying down. I also have dizziness, brain fog, and severe fatigue probably related to this. Brain MRI scans show no issues. Has anyone had this issue, and what was the cause and solution?

Glad to hear so much has been ruled out. rnlorena posted an interesting plan she will be seeking. Yes, sometimes one must travel far to find medical answers no one seems to know in your location.
Remember, there are many, many neurological disorders...some most people have never even heard of. You might fall into this category. The CSF leak usually presents as no "headache/pressure" when lying down. Upon rising, the pain does not start immediately, but will come. I also had pressure on my temple and front of my head and my neurologist thought of a CSF leak which I did not have. Also, remember there are many "types" of migraines. If I were you, I would look at US News & World Report for 2025 medical center rankings. Look at who is #1 in the country for neurology. That's exactly where I would go for another opinion.

You said that you also had this problem but a CSF leak was ruled out. What was the cause of your problem, and how was it resolved?

The cause of my head pressure was simply diagnosed as migraines. I have refractory migraines which means very difficult to treat. Migraines take many different forms...believe me, I know. It was never resolved but unlike you I do not have the pressure daily. Since I have heart disease, I can not take any triptans, only the CGRPs which I have discovered are really ineffective for me. For months I had no prevention medication to take. Candesartan was started since this a 2-for-1. Blood pressure control and migraine prevention. It has been somewhat effective. I also have "periodic brain spasms" which, if they lasted more than 20 seconds, it would be a 911 call. Beyond painful in my head and frightening. I will see my neurologist in November, don't know what's up his sleeve next for me. I am still paying the price of my 2019 concussion and it's not improving. I have been to several of the top neurology centers in the country, either by car or a plane flight.
Bottom line? No one has any magic bullet and b/c I can't try the triptans it makes my situation worse. My guess is "maybe" the triptans would work for me but I will never know. Ice packs are almost my constant friends. At this point, I simply take Tylenol ES with some relief. And b/c I have a severe panic disorder I also will take Xanax when it's very bad. And simply and totally relax, completely zone out, staring into space. I often go to bed wearing one of my many ice packs I have collected. I certainly feel for you. I still think you need to explore your constant head pressure at a top-notch facility. And as I said, there are many neurological problems we have never even heard of...some quite rare. You might fall into this category. All the best to you. Hopefully you will find answers...please keep in touch with us.

xavier99 I got very lucky today. I have a morning appt on 8/20 for a neurologist that deals with things I have experienced. I was out and about with my brother today and I got a phone call. I now have an appointment to see a Neuro-Opthalmologist on 8/20 in the afternoon. They originally made me a first available for September 29th and I told the girl that I had been going through this head pressure for 6 months. I told her I drove from Florida and it would be hard for me to be gone that long. ( If push comes to shove I would do it) Thankfully I will see both doctors on the same day. I will let you know what the neuro-ophthalmologist says about head pressure and your eyes and vision. I will be prepared for both visits. @rnlorena

I hope your appointment went well yesterday. I also have head pressure at the temple/top. In addition to that, it feels like a moving signal/sensation as well, and it causes headache with pressure early mornings at 3 or 4am and often wakes me up.

I saw my primary care doctor and she ran all the regular blood work and nothing was found. I also had my MRI and saw a neuro and she said it is probably a migraine.

Later this month, my primary care doctor ran B - Panel and found that my B6 level is high (Normal - 2 - 20) mine was 40. I have a followup with her in the second week of Sep.

It looks like high B6 is definitely not good for brain or nervous system and I am trying to see how to reduce my B6 to normal levels.

I have a similar thing and so far no clues except one thing - my primary care doctor ran vitamin B - Panel and found that my B6 level is elevated (Normal - 2 - 20) mine was 40. I have a followup with her in the second week of Sep.

It looks like HIGH B6 is definitely NOT good for brain OR nervous system and I am trying to see how to reduce my B6 to normal levels.

Did the pressure start after the MRI? I have had pins and needles in my head since my 1st MRI! JJ

Your symptoms are similar to mine. MRI and blood tests normal except I do have follicular lymphoma diagnosed in lymph gland near sight of constant pressure/pain on right side of my face impacting ear, eye, neck and head. No diagnosis from neurologist but my GP and one other doctor believe it may be Trigeminal Neuralgia. Treatment is Lyrica, but it is slow to kick in and causes brain fog. Eating, singing, talking and group interaction trigger more pressure. I have no pain at night. If anyone reading this has recommended treatment, I would be grateful and perhaps it would be helpful to the one posting the original message. Thanks, Joan

I had a lumbar puncture done yesterday and the pressure was 21 cm H2O. My doctor said that because it's over 20 cm I have IIH, and I need to be treated with something like Diamox. But to me, just going over the normal range by just 1 cm doesn't explain the severe symptoms I am having as I explained in my original post. Has anyone had severe IIH symptoms with pressure in the low 20 cm range?
