Focal awareness seizures or auras?

My daughter has her neurologist thinks are awareness focal seizures. They are very short (5-10 seconds) and happen during stressful situations, when experiencing anxiety, and during and around her menstruation. Her doctor put her in keppra 250 twice a day and it didn't change much. Then it was increased to 500, then 750 and then 1000 mg twice a day. She still has an occasional aura feeling but not sure if its epilepsy. She is having side effects from meds; tired, crabby, etc and does not want to take drugs she may not need. She was going to ask to slowly wean to 750 twice a day and see if there are any changes. Has anyone had a similar situation?

I’ve been in it 2 weeks
I am taking 100 mg in the morning. Started out with 100 mg at night. Made me anxious and couldn’t sleep.
Prescribed by my pcp.
So far no seizures. 🙏

I've also experienced sleep issues with several AEDs I've tried. Many doctors find that switching the timing of medication from evening to morning can reduce or even eliminate this side effect.
Did switching your Gabapentin to morning help improve those sleep-related side effects?

I was diagnosed with Focal seizures at 78 years old! I had 5 of them in a year. Then put on Keppra 500 mg twice a day. I have never had another one and my Mayo Clinic Neurologist reduced the am one to 250 mg about 7 months ago. I still sense the Aura once in a while but never have had the 3 minute seizure. I am tired from the medication and going to ask the doctor when I see him next week if I can lower the Keppra at pm to 250mg.

Getting an epilepsy diagnosis with focal seizure at 78 must have been quite unexpected, but it sounds like you've been responding well to Keppra. 🙏
When were you diagnosed with epilepsy and how long have you been on Keppra?
You mentioned that you are tired of the medication. Are you experiencing any side effects from it?

I have focal aware seizures in my right temporl lobe. I have tried the following medications: Briviact, Carbamazepine, Lacosamide, Lamotrigine, Keppra, Aptiom, and Clonazepam. I am currently on 250 mg of Xcopri-Cenobamate. I tried to go up to 300 mg, but it made me extremely sleepy, and I didn't feel well. I also take .5mg of Clonazepam if I have several seizures in 1 day.
Has anyone had any success with a medication I haven't tried?

I tried several different seizures drugs with little or no relief. I went though 6 neurologists until one suggested I try using both lamicatal and trileptal. They are similar but not identical drugs and, for whatever reason, the combination of the two worked for me.

Hello,
I've been on Xcopri since October 2023. I started with 150mg. I've been on 250 mg for a few months now. I've often thought my seizures may be triggered by my hormones. I am menopausal now. I go to the EMU Sept 2nd. It seems strange to pray to have a seizure, but I pray I have them while I'm in the EMU. I have not tried Gabapentin.

My neurologist and gynecologist have worked together to develop a hormonal therapy that has significantly reduced my hormone-related seizures. I'm currently in perimenopause and continue with the hormonal treatment, which remains highly effective.
Have you had the chance to speak with both your neurologist and gynecologist about this? Also, has your EEG been scheduled to coincide with a specific phase of your menstrual cycle?

She has had a couple EEG's and an MRI and nothing showed up. We are seeing a neurologist at Froedtert Hospital in Milwaukee. We are in the process of trying to figure out if something else is going on. Thank you for your reply! I feel like her Keppra dose is high for the symptoms she was having. We are going to discuss with Doctor but sometimes they are so stuck on protocols.

Hi Chris,
My daughter has been trying to contact her neurologist about how to wean off her keppra. He told her if she doesn't have any focal seizures in 2 years, she can wean slowly. The nurse has not responded to any of her messages. She was on 1000mg twice a day. She went to 750mg for 2 months and didn't have any problems, and then 500 twice a day and its been 5 weeks and still no change. After 2 months she will drop to 250 mg for another 2 months. Overall, she feels better as she weans. She is exercising regularly and being careful in the way and what she eats. Eats a very limited amount of sugar. we are trying to find a new epileptologist to work with. It seems as though every neurologist just places you on keppra and doesn't really listen to what you are telling them. Thanks for checking in. My daughter is an adult so I have to trust she is doing the right thing.

Yes. I take 100mg in morning. I am sleeping fine
No seizures. But occasionally I get a weird feeling. Can’t describe it. Happens late in afternoon. Probably after Gabapentin wears off.

Hello Chris,

I have not had a menstrual cycle since December 2024, but I do have the long term EGG schedule when I usually have the seizures. They are usually the 1st week of the month. I will definitely look at the two resources you shared.

I'm so glad to hear that your daughter is doing better! I hope you'll find an excellent epileptologist for her care soon. 🙏 I understand that her appointment with the neurologist at Froedtert Hospital was somewhat disappointing?
As @adoptivemother once mentioned, it might be worth having PNES evaluated as well. Early this August, I experienced my first PNES episodes following a complex partial seizure. What surprised me was that the PNES episodes diagnosed at the hospital felt exactly like my usual auras. Through this experience, I learned that it's possible to have both epileptic seizures and PNES episodes. How is your daughter doing with those "small auras" since her Keppra reduction? Has there been any change in their frequency or intensity?
