why is this happening to me
I’m a 31-year-old male (180 cm, 110 kg) with anxiety, panic disorder, and derealization since a sudden panic attack on March 11, 2024. Medical tests were normal. I tried cannabis once (2 months prior), had a panic reaction, and never used it again. Diagnosed with depression and GAD. Treated with citalopram (up to 40 mg, partial response), then bupropion (up to 300 mg). Panic attacks stopped after 6 months, but anxiety, insomnia, depersonalization, and intrusive suicidal thoughts remained. Citalopram was stopped; started sertraline 50 mg in Dec, increased to 150 mg in March. Bupropion was later stopped. After 4 months on sertraline 150 mg, I still have intrusive suicidal thoughts, derealization, identity confusion, and emotional detachment. I live in social isolation, gained 25 kg, and take supplements (Omega-3, magnesium, D, K2, B12, probiotics). I do CBT exercises, but relapses continue.
Should I stay on sertraline? Do I need augmentation or a med change?

Hi,
You need to talk to your doctor about the antidepressants and how they may be affecting you. Also since you stopped taking your vitamins do a search for the foods that would have those vitamins and minerals in them and eat those foods that you like. Natural foods with vitamins are better for you than taking vitamins in a pill form anyway. Your body is dealing with some issues and it needs all the help it can get.
Don't even consider suicide. Suicide doesn't always work. You could just end up paralyzed for life or very sick. When you have those suicidal feelings just ask God for help. You don't have to make a formal prayer. Just say, "God, please help me!" God knows all about you and your problems. He made you so he can fix you. Also he loves you and doesn't want you hurting yourself.
You sound like a very nice person! I'm glad you found this website. People on here are very helpful. I know they helped me a lot when my husband died a year ago.

I know as a Mom, I would want to know if my son was having some problems like you are. It's good that you don't want to have thoughts of suicide and you are trying to control them! That is proof that you are a good person and will be OK! Also, you are only 31. You have your whole life ahead of you with all the wonderful things to come in it!
