Skin problems under Ostomy appliance
I have a urostomy since Feb 2022 and have a lot of problems with the skin under the coloplast appliance. I have done allergy testing and I am not allergic to the appliance . Does anyone have this problem and what helps heal the skin?

Can you share what the paste is?
Also what’s the advantage of a two piece?
Folks use the paste to fill in the dips in their anatomy which are situated under the barrier/flange.
I have a very deep navel cavity which is a problem for me leak-wise. I used Coloplast's Brava® Ostomy Strip Paste to fill it in until I came up with a better idea. I cut out a small piece which covers my cavity from Convatec's DuoDerm. The stuff is costly to buy but I'll be gone from this world before I can use it all up. It's no mess, no fuss, works beautifully.
As for the two piece system. I use Coloplast's Sensura Mio 2 piece. Perhaps just my preference. I can take the pouch off the flange to shower.

2 ReactionsI use a Hollister two part system. Before I apply the flange, I apply calamine lotion all around my stoma area. Hit it with a blow-dryer until it is chalky dry, then apply skin prep and then the flange. Warming the flange with the blow dryer helps with adhesion for me. Recently I bought a stoma cover for showering on temu. for about $10.00. Use without the bag to cover the flange when showering. I have lived with an ostomy for twenty years. You can do this!

2 ReactionsYes, I can describe the paste, it is called Hollister Ostomy Adapt paste, the Hollister number, is 7906. It comes in a tube and I run a bead of the paste around my stoma before attaching my New Image flange to my skin. Full description below. I also added a picture of my four products that I use.
1. This is the steps I do for each new attachment.
2. I wash my skin with soap and water making sure all soap is off my skin.
3. I then clean it with alcohol swabs usually two to make sure I get all the bacteria off my skin, (I don't touch my stoma with the alcohol) so when putting a new appliance on I don't have any bacteria on my skin that will break down my skin.
4. I then use Hollister Adapt Stoma Powder, I squeeze it around the stoma.
5. I then run a bead of Hollister Ostomy Stoma Paste around my stoma.
6. I then attach the flange to my skin. This way I have the stoma paste as one more defense of attachment so the bag doesn't fall off.
I have done this from the first month I got my colostomy. My surgery was April of 2017 and I have never had any trouble with my skin breaking out getting ulcers under my flange or even a rash.

1 ReactionThe advantage of a two piece is that I don't have to change my flange that adheres to my skin for every new bag. (which I feel causes the skin to break down with so many changes.) I can rinse the bags out or throw them away but I rinse them often. I get about 7 to 9 days out of each new placement. I don't have to buy so many bags because I do rinse them out and get more days out of them. If I had a one piece I would have to change the whole set up each time I needed to change the bag, which would be for me every day. I have written a long explanation about the paste in a previous note.

2 ReactionsI use the Convatec two peace appliance. To protect the stoma I put a thin layer of Eakin cohesive paste around the stoma. I use Esenta skin barrier adhesive wipes. I then apply a light dusting of Nystop powder brushing off excess powder or you can use the used adhesive to dab off the excess powder. I then apply the moldable skin barrier to fit my stoma. Snap on the bag and warm the skin barrier by warming a package of disposable wipes in the microwave for 15-20 seconds, then hold it on the appliance for about a minute this helps to adhere the wafer to your skin.
When I travel and don’t have access to a microwave I’d use a hair dryer. No leaks for me. You need prescription fo the Nystop

2 ReactionsOur Ostomy Nurse at Mayo Jax suggested we use Flonase. That was 4 years ago and although my husband’s skin is an angry red color, it has never had any sores or infections. I don’t know what optimum so maybe someone can enlighten us. He dries the area with a cool hair dryer before and after applying the Flonase. He uses a one piece pouch and changes it out once a week. I hope this helps.

2 ReactionsI choose to use a coloplast mio flex one piece closed end pouch which is disposable I feel much cleaner having a clean bag on every time It takes me all of 5 mins to change the bag I do not use a lot of products since their products can be used without skin barrier I simply wash area with warm water dry with hairdryer and attach new bag I never have skin problems I love to soak on a hot tub every day and thoroughly clean stoma area When I do go out I will use skin tac and barrier strips for added protection but I dont at home If i feel any irritation I use stoma powder but that is rare I was having a problem with pancaking and saw a great stoma nurse at Mayo in Jacksonville and she gave me helpful tips about placing a blue sticker over the air filter and making sure there is some air in the bag before you attach it and I have not had an incident since. This is now a way of life for me and I simplify the process and number of steps required to change my appliance When i tried using systems where I only changed the wafer every three days I got irritated and itched Everyone has to find what works for them but I wont let this consume my life I see no need to wash bags or over prepare the stoma area that is a waste of my time I just make this as simple a routine as I can

3 ReactionsI have a colostomy, and have had it sense 4/2017, I clean my skin with an alcohol swab or two before putting the face plate (flange) o that way all bacteria is gone. I have never had a rash or an ulcer my skin is nice and pink when i change appliances. This is the only thing I can suggest to keep bacteria from growing under the flange.
